Today, I am blessed to have here with me Dr. Brian Lenzkes. He is an internal medicine doctor based in San Diego who has focused much of his attention on clinical nutrition. Dr. Lenzkes attended UC Irvine and received a Bachelor of Science in Biology before attending the University of Southern California Medical School. [01:10] Learning About Fasting and Low-Carb In The Medical Community Throwing drugs at people with diabetes, hypertension, and obesity doesn’t do anything. One patient found fasting successful, and Dr. Lenzkes decided to research the lifestyle. After losing weight with fasting and low-carb, Dr. Lenzkes was hesitant to share this new information with his patients. Dr. Lenzkes got a lot of backlash from his colleagues. However, Dr. Lenzkes gets patients off insulin; not many people can say that. [13:30] The Correlation Between High Insulin Levels and Cardiovascular Disease There's a massive correlation between high insulin levels and cardiovascular disease. Nearly 100% of Dr. Lenzkes’s patients have never had an insulin check until seeing him. You don’t just wake up with type two diabetes; you have ten years to predict that diabetes is coming before you get it. [19:40] How Exercise and Movement Will Help Prevent Type 2 Diabetes Most diabetics, if you look at their lab work untreated, they're going to have high triglycerides in that 300 to 500 range. LDL can be increased or low; it doesn't matter. HDL will be low. When you get HDL right and the triglycerides come down, you feel a lot better. Exercise and stress management is essential because it helps burn sugar and your fat reserves. When you run out of reserves, you have diabetes because the sugar stays in your bloodstream. [31:50] The Reason Doctors Don’t Talk About Diet and Lifestyle During A Visit Dr. Lenzkes uses a company called Ulta Lab Tests: https://www.ultalabtests.com/. Patients can order Ulta, or your doctor can get it for less. Doctors have been trained a certain way, so they may not want to test for insulin. Plus, doctors are pressed for time and are judged based on how many patients they see each day. Many doctors only see a patient for nine minutes. It is impossible to talk about diet and lifestyle during these visits. [36:10] Many Doctors Don’t See The Truth; Instead, They Stick With What They Learned In Medical Schools Doctors are taught that you need three meals daily, and breakfast is the day's most important meal. However, that’s simply not the case. Doctors stick with what they learn no matter what evidence says otherwise. Many doctors will see high blood pressure and tell people to cut salt. Well, salt is high because insulin is high. High insulin causes you to retain sodium in your kidney. The answer should be to lower insulin instead. [42:50] Lower Your Insulin Through Sleep, Exercise, and Stress Management What’s more important to lower insulin, sleep or exercise? If you prioritize something, it’s sleep. Next, don’t be stressed out and angry. Then, eat real food that isn’t processed to lower insulin. Regular exercise will also help lower insulin. The more muscle mass you have, the more insulin sensitive you are. Putting muscle mass on is never going to be the wrong answer. The more muscle mass you have, the longer you live. The lower your insulin levels are, the longer you live. [57:30] Chronic Stress Could Be Causing Your Metabolical Sickness Fatty liver disease means you are metabolically sick. When you are metabolically sick, you risk dying from all different types of diseases. The biggest scare we have is high insulin. When you’re under chronic stress, it’s not good. After getting health insurance, Abel found a traditional doctor. After taking his advice, he gained thirty pounds and suffered from high blood pressure. Abel started researching physiology and bodybuilding. He began eating loads of fats and vegetables. No longer was Abel afraid of salt. After taking it seriously for 30 days, he had a different body composition. It made him mad because he had been trying so hard to eat “right.” People need to be educated about the fundamentals, which is why Abel started The Fat Burning Man. Abel did almost precisely the opposite of what his medical doctor was telling him to do.   [09:30] The Fundamentals    If you want to be strong, you have to be balanced and focus on mobility. Do not isolate your biceps to get big biceps. For 99.9% of people, it will make no difference to do isolated exercises. Strength training your big muscles group will work. Kettlebells, free weights, and using the correct form will assist with your fitness. It doesn’t need to get much more complicated than that. Eat clean plant and animal foods. Ditch white sugars – it’s incredibly addictive and destroys our health. Find diversity and balance in your diet. Make sure you’re getting enough protein and fiber. Do not get vegetables that are sprayed with chemicals. Practice common sense with technology. Our cell phones can cause cancer and tumors. Yet, people still keep their phones in their pockets.   [16:30] About The Wild Diet Abel wanted something that would be offline and lead people to the truth. Which is why he wrote this book back in 2015. There is not a lot of things that are truly out there. Many authors and podcasters like to make things overly complicated – it works! You can sell a lot of stuff that way. Abel had been listening to the wrong people; it hurt him. The Wild Diet is written simply and should be easy to read.   [20:15] When Should You Eat Carbs?   It depends on what you are doing with your life. Eating carbs in the morning may drive you crazy. You will have cravings and be hungry for the rest of the day. If you are going out and running a marathon, you could easily eat carbs in the morning. Abel drinks tea, coffee, heavy cream, and coconut oil. He will also drink bone broth and powdered greens. Dinner will be his first and only meal of the day. Eat fibers, proteins, and fats before you hit the carbs. Slam down an orange juice and see how you feel in an hour. It takes a few weeks for your body to realize that you don’t even need orange juice in the morning.   [23:50] Exercising in a Fasted State   Abel runs in a fasted state. When you run out of sugar in your muscles, you are going to bonk out. Learn how to manage that, and your body will adapt. Most people are carrying too much extra weight – so exercising while fasted will not be harmful. Fasted cardio will help you get the weight off pretty fast. However, it can be brutal at first. Depending on your goals, it can be helpful to eat before working out. When you are training fasted, you are getting the advantage of not needing to fuel.   [29:55] What Does “Natural Flavor” Mean? Natural flavors might mean the extractives of wood products or anal beaver glands. Natural flavors are usually better than artificial flavors. Flavor your food with actual fruit or honey instead of artificial things. Do not be generous with your trust at the store. What is actually in the food you are buying?   [33:00] Weight Loss Tips Focus on the foods that are your trigger foods. Try and go without this food for a few days and then for a few weeks. Go without that food until you don’t have those cravings anymore. It could be pizza, cookies, and cakes. Sometimes it’s not about the food that you are eating, but about the state of mind you are in while you are eating. If your attention isn’t with your food, then it’s harder to realize when you’re hungry. It doesn’t take long to eat, so sit down and practice gratitude.   [38:35] Abel’s Gas Leak Scare Carbon monoxide leaked in Abel’s rental while they slept. Abel, his wife, and their dog were overcome with apathy. The effects of the gas make you dumber and less likely to make the right decisions. Abel and his wife were close to that state. The gas is odorless, so it’s almost difficult to detect. They almost died, and that is not an exaggeration. Abel was not able to work for months. Carbon monoxide detectors could easily prevent something like this happening.   [44:45] Recovering From Carbon Monoxide Poisoning Abel did long-term fasts for his body to heal. Do not push your body too hard too soon. You want to be active in your recovery. However, be honest with yourself and be conservative. Vitamins in an IV drip brought Abel back to life – it’s an effective way to get nutrients into your blood stream. During the drips, their inflammation would immediately come down. Eating clean and not drinking alcohol was also extremely helpful. Today, I am lucky to have here with me, the CEO and co-founder of the Fasting Method Program, clinical educator, and researcher, Megan Ramos. Megan’s major role in the The Fasting Method is to facilitate the patient’s education on the Fasting Method dietary principles and counsel them on their fasting regimens. Megan, along with her colleague Dr. Jason Fung, believe that both what you eat and when you eat matter. The Fasting Method utilizes therapeutic fasting and time restricted eating protocols with their patients to help them reverse their type 2 diabetes and achieve sustainable weight-loss. They encourage their patients to eat real foods and adopt a low-carb lifestyle in addition to the fasting. Megan was diagnosed with non-alcoholic fatty liver disease (NALFD) at age 12, polycystic ovarian syndrome (PCOS) at age 14, and type 2 diabetes (DMII) at age 27. She reversed these metabolic conditions and achieve an 86-pound weight loss by incorporating intermittent fasting with a low carb diet, and is now passionate about helping others do the same. Megan lives in Toronto, Canada. She is a director of the nonprofit organization Public Health Collaboration Canada (PHC Canada), works on the editorial board of the Journal of Insulin Resistance and Team Diet Doctor. In this episode, Megan Ramos opens the show explaining her various health problems growing up. She started working with Dr. Jason Fung at the age of fifteen, and together, they realized the importance of fasting. She was diagnosed with fatty liver disease, polycystic ovarian syndrome, and type 2 diabetes. She was skinny, but all of her weight was fat. Her doctor couldn’t tell why she had diseases of obesity. Megan didn’t stress about it. “Your genes aren’t your destiny.” –Megan Ramos Megan started working with Dr. Jason Fung at age 15. Her research patients were trying to live life right; however, they were just gaining more weight. Megan also was gaining weight despite hiring a nutritionist and personal trainer. When Dr. Fung would introduce fasting to his patients, they would feel better. It was the missing piece of the puzzle. Megan started fasting, and it finally all made sense.   [13:10] Symptoms for Kidney Dysfunction Patients who are leaking a lot of protein in their urine, yet they don’t have high blood pressure are most likely suffering from kidney dysfunction. Protein in the urine usually means something diabetic is going on. “In North America, you have insulin resistance until proven otherwise.” –Megan Ramos Even though your labs say normal, a conventional doctor will not be able to read the labs as an expert would.   [17:30] Being Proactive Rather Than Reactive Megan wants to help patients be proactive. The healthcare system is broken beyond repair. When Megan and Jason first started talking about fasting, people thought they were lunatics. However, the benefits of fasting have been well known for hundreds of years. Fasting makes a significant change in the lives of their patients.   [23:40] Fasting the Wrong Way Fasting the wrong way will result in electrolyte imbalance. Most people are petrified of salt. However, salt is critical to our survival and wellbeing. Insulin levels will be high when you first start fasting. Eventually, the insulin levels will tank. It could cause diarrhea if not rehydrating correctly. Megan does consultations online. Most of the time, the person’s issues are resolved by salt or magnesium. Pickle juice and bone broth will also assist with hydration. Sometimes people carb up before a fast. It will wreak havoc on your body and mind. Plus, you won’t get autophagy as efficiently. Do a fast through your ketogenic diet. Fasting shouldn’t make you feel bad; it should make you feel more energetic.   [31:40] The Most Surprising Benefit of Fasting Megan loves when older women get their libido back. The most surprising thing is anti-aging in women. Women who have gone grey are now getting their hair color again. A 76-year-old woman got her regular menstrual period back for two years.   [33:50] Should Women Practice Fasting Differently Than Men? Women need to be more diligent with sodium. Salt is critical in middle-aged women. Not necessarily for feeling good, but for making sure that weight loss is happening at an efficient rate. Women need to fast more than men to get the same results. A 24 hour fast will give women tremendous results. They will lose water weight and body fat. Women are more compliant doing a three day fast. Once they wake up on the second day, there should be no hunger. Staying in a fast can be more manageable for women.   [37:55] Advice for Someone Who is Getting Stuck at a Certain Weight Make sure there are no problem foods or snacking in the diet. People on keto will eat a lot of nuts and cheese outside of eating windows. What do your eating days look like? Add fat and protein to your meals to make them more satiating. That alone can break a person through a plateau.   [42:55] Megan’s Favorite Benefit of Fasting Megan gets an insane amount of energy while fasting. She gets to train doctors and speak at medical conferences. After a trip, Megan does a fat fast.
