It will cause you to burn excess body fat and improve your overall health. But there’s one important caveat. A constant, unrelenting state of ketosis—in which your body is burning its own fat for fuel—is obviously unsustainable, because eventually you’d have no more fat. In this chapter, we move into the Flex pillar, which will allow you to maintain your Keto lifestyle month after month, year after year. Four Reasons Why Prolonged Ketosis Is Counterproductive My mantra is, “Lose the excess fat; treasure the necessary fat.” Ketosis is a brilliant strategy for accomplishing the first goal. As for the second goal, prolonged ketosis doesn’t have an automatic shut-off valve. If you remain in state of constant ketosis, burning your own fat, then your body will keep doing that as long as it must to stay alive. If necessary to preserve life, once your fat is gone, various bodily functions will be shut off or minimized. This process of shutting down will continue until Mr. Death knocks on your door and says, “Sorry, buddy—time’s up.” No one wants that. To ensure that you’re well informed, let’s review some of the bad things that can happen if you go too far with ketosis. Fat Burning Slows Down As levels of body fat drop, your primal brain, whom we thought we had trained to accept a high-fat diet and fasting, will be aroused and will begin to object. Why and when will this happen? Your primal brain, that eternal watchdog, will sense that your reserves of fat are becoming depleted. Remembering those long periods of deprivation thousands of years ago, your primal brain will sound the alarm: The fat supply is running low! It will issue the command that your body must preserve its precious fuel. In response, your body will actually slow down fat burning—all for the sake of survival! When you stay in ketosis too long, the body starts to rebel, and conserve its fuel. A good analogy is if you lived out in the woods and you had stored 500 logs of firewood to heat your cabin for the winter. These were your reserves, because you could still go out and chop more firewood. So if you had to dip into these reserves, you wouldn’t be worried. But now it’s early March, and snow still covers the ground. It’s been a tough winter and you haven’t harvested many trees, and you see that you’ve got only 50 logs left. That’s a very low supply! Suddenly you’re reluctant to use them because you don’t want to run out. When you had 500 logs, you didn’t care about burning them. The same thing happens with your body fat. This is why when you stay low carbohydrate for too long, your body will slow down fat metabolism. And you know what? This is a good thing. There is a big difference between losing excess fat and losing all your fat. You want to lose excess fat. But your body needs a small amount of fat, and your primal brain is smart to insist that you meet this minimal requirement. Thyroid Problems Hormone production is often a function of a “chain of command,” as orders are passed from one organ to another, until the desired outcome is achieved. The hypothalamus and the pituitary gland, which are located in the brain, help control the thyroid gland, a butterfly-shaped gland located at the base of your throat, just below the Adam’s apple. To provide their direction to the thyroid, the hypothalamus releases thyrotropin-releasing hormone (TRH), which stimulates the pituitary gland to release thyroid-stimulating hormone (TSH). When the hypothalamus and pituitary are working normally, they respond when: Thyroid hormone levels are low, so they secrete more TRH and TSH, which stimulates the thyroid to make more hormones. Thyroid hormone levels are too high, so they secrete less TRH and TSH, which reduces hormone production by the thyroid. The thyroid gland uses iodine from food to make two thyroid hormones: triiodothyronine (T3) and thyroxine (T4). The primary secretory product is inactive thyroxine (T4), which is a prohormone of triiodothyronine (T3). T4 is converted to T3 peripherally by tissues with high blood flow, including the liver and kidneys. In the brain, T4 is converted to active T3 by type 2 deiodinase produced by glial cells. The thyroid stores T3 and T4, and releases them as they are needed. These two thyroid hormones function in almost every cell in your body, and play a fundamental role in regulating your metabolism and maintaining your resting energy expenditure—that is, the energy you burn just to keep functioning while sitting on the sofa. Diet can influence thyroid hormone levels, and prolonged ketosis and severe calorie restriction can lead to reduced thyroid function, or hypothyroidism, which is a mechanism to ensure survival in response to perceived famine. The goal is to slow metabolism, preserve energy, and preserve vital organs. T4 is the inactive form of thyroid, it needs to be converted to T3 which is the active form used by the cells. Do you know which hormone helps with this conversion? Insulin. Low thyroid hormones cause a drop in levels of insulin, the hormone needed to transport sugar from the blood to different cells throughout the body. Chronic low insulin levels slow down cellular function, including muscle contractions and basic brain function. Over longer periods of time, low insulin levels compromise this T4 to T3 thyroid hormone conversion. Testosterone Deficiency Sex hormone binding globulin (SHBG) is a protein made by your liver. It binds tightly to three sex hormones found in both men and women—estrogen, dihydrotestosterone (DHT), and testosterone. SHBG carries these three hormones throughout your blood. SHBG controls the amount of testosterone that your body tissues can use. Too little testosterone in men and too much testosterone in women can cause problems. Factors such as sex, age, obesity, liver disease, and hypothyroidism can change the level of SHBG in your blood. Hypothyroidism is when your thyroid produces an insufficient amount of thyroid hormones, which can happen with prolonged ketosis. What I’ve seen reviewing lab work over the years is too much SHBG leads to lower testosterone levels, and not enough SHBG leads to an increase in estrogen; both can be problematic for men and women. Long term ketosis can lead to either of these. Leptin Reset Leptin is your “stop eating!” hormone that plays an instrumental role in your hunger and weight management. It comes from the Greek word ”leptos,” meaning “thin.” Fat cells produce leptin in proportion to levels of body fat: the more fat cells you have, the more leptin they make. When your body fat increases, your leptin levels go up. Your brain senses the increase in leptin, and thinks, “Okay, we’ve got enough fat in reserve, so we can stop eating.” Thus, you eat less and burn more. Conversely, when you don’t eat, your body fat decreases, leading your leptin levels to drop. Your brain, sensing a shortage of leptin, thinks, “Hmm, we’re low on fat reserves. Better find some food!” At that point, you eat more and burn less. This negative feedback loop is similar to the control mechanisms for many physiological functions, such as body temperature, breathing, and blood pressure. A properly working leptin system tells your brain that you have enough energy stored in your fat cells to engage in normal, everyday metabolic processes. It leads to better brain function, memory, metabolic performance, mental sharpness, and coordination, and it can even affect the regulation of mood and emotion. This is wonderful—except that many overweight people suffer from leptin resistance. They have plenty of blood leptin, but the brain doesn’t recognize it. Despite the abundance of leptin, the brain thinks there is too little leptin—and orders the body to keep eating! This is obviously a bad situation. What can cause it? Several potential mechanisms behind leptin resistance have been identified. Free fatty acids. Having elevated free fatty acids in your bloodstream may increase fat metabolites in your brain, and interfere with leptin signaling. Inflammation. Inflammatory signaling in your hypothalamus could be a cause of leptin resistance. Having consistently high leptin. It may be that your brain gets accustomed to a constant flood of leptin churned out by your fat cells, and becomes “numb” to it. So, what happens when you follow a low-carb keto diet that puts you in long-term ketosis? Perhaps paradoxically, many people do not suffer from an increase in appetite, as you might expect from falling levels of leptin. It’s complicated, but the short version of the story is that leptin levels will drop, which would normally signal your brain to start eating. But something else happens—your brain becomes more sensitive to the effects of leptin, so a smaller amount of leptin packs more punch. In addition, the brain’s response to leptin is inhibited by inflammation, resulting in increased leptin resistance. Obesity increases inflammation, which increase the brain’s “numbness” to high levels of leptin. Inflammation is dramatically reduced by sustained nutritional ketosis. The reduction in leptin resistance due to reduced inflammation adds to the brain’s heightened sensitivity to leptin. In other words, on a well-formulated ketogenic diet the brain perceives a greater satiety response to less leptin. This explains the paradox as to why we see a decrease in appetite with nutritional ketosis. In order to achieve a sustained state of ketosis, you need to occasionally boost your levels of leptin and send the signal to your brain that higher levels of leptin are normal, and that you need to keep eating. He lives in Salt Lake City, Utah, with his daughters, who remind him every day not to take things too seriously.   In this episode, Drew opens the show explaining his Fit2Fat2Fit movement. Drew has always been in phenomenal shape – he couldn’t understand what his overweight clients were going through. In order to understand, Drew gained 75 lbs and lost it again. Drew talks about the difference between burning sugar and fat and why he ultimately decided to follow a ketogenic lifestyle. Stay tuned to hear Drew discuss gratitude and how to avoid the keto flu.   [04:00] About Drew Manning Drew had never been overweight in his life. He couldn’t understand his clients – they wouldn’t go to the gym and wouldn’t eat healthily. Drew decided to gain weight to understand his clients better. At first, gaining weight was fun. Cereal and soda taste amazing. The first few months were easy. Then, it shifted. Going up the stairs was difficult, and he started snoring. Plus, Drew was greatly affected mentally and emotionally. He didn’t know who he was when he was out of shape. It opened his eyes, and helped him find his meaning. When Drew switched back to eating healthy foods, he felt miserable. Drew suffered from headaches, a severe lack of energy, and mood swings.   [12:15] The Fit2Fat2Fit Movement In 2011, Drew started with zero connections. A fitness blogger posted his story, and Yahoo picked it up. Drew’s website crashed because all sorts of TV shows were trying to get ahold of him. Eight years later, Drew is still talking about his story and has two seasons of his TV show.   [16:10] Why Drew Chose the Ketogenic Lifestyle Drew likes to do experiments on his body. He found out there were all sorts of scientific research on the ketogenic diet. Drew did a sixty-day experiment on himself. He didn’t lose any weight, fat, or muscle, but he felt amazing. Drew went from eating six meals a day to eating two meals a day. His brain has never felt so mentally clear. Plus, his digestion is better. No longer did Drew feel like he was a slave to food.   [21:15] The Difference Between Clean and Dirty Keto   When Drew first started the ketogenic lifestyle, he simply cut carbs. It was an uneducated form of keto. There are different levels of keto. You can keep taking amazing steps – Drew is always changing, evolving, and upgrading. Drew uses a whole foods approach. In America, there are convenient keto options, even though they are not the cleanest ingredients. Dirty keto has evolved and become more popular. Drew suggests people stop eating nuts and dairy if they are still feeling inflamed. “Just because you are in ketosis, doesn’t mean you’re healthier.” -Drew Manning   [28:45] Following Keto Without a Gallbladder   People without a gallbladder have a hard time digesting fats in a keto diet. Drew found an enzyme that will assist most people with their digestion. Gallbladder removal is the number one surgery in the United States.   [30:00] The Difference Between Burning Sugar and Burning Fat Most people are sugar burners because they have cereal for breakfast, sandwiches for lunch, and pasta for dinner. When you eat fewer carbs, your body will retain less water. When you retain less water, you miss out on essential minerals. Therefore, when you transition to the keto diet, you will experience the keto flu because of an imbalance of electrolytes. When Drew says electrolytes, he does not mean Gatorade! Drew tells people to take sodium, magnesium, and potassium. Plus, you need to be drinking a lot more water.   [32:50] The Habit of Practicing Gratitude “Anytime you hit rock bottom, you become a better version of yourself.” -Drew Manning People overlook the importance of overcoming mental challenges to make keto a lifestyle change. Gratitude is a daily task that Drew asks people to do. If you write three things that you are grateful for, then throughout the day, you will look at situations in your life differently. This pain was the beginning of the four and a half years that Ashleigh fought for her life. She was a mystery patient, no one quite knew what was going on, and Ashleigh was sent home to die. Ashleigh’s metabolism was so screwed up. Her body did not know what to do with food; she ended up gaining sixty pounds while eating nutritiously. Ashleigh hated her body and felt like she needed a new one. However, she realized she wouldn’t be able to heal if she hated her body. Her body is the only thing that is keeping her alive. Recognize the incredible gift of life that your body gives you. “It came down to reverence for life first.” -Ashleigh Di Lello   [18:50] Practicing Love and Gratitude Our subconscious does a majority of our thinking. The older we are, the more it can take over. Say affirmations like “I love my body.” These mantras will help fight the subconscious thinking. Another thing you will need to do is be more aware of those subconscious thoughts. When is the first time you decided that you hate your body? Realize that you can change those thoughts in your head.   [26:45] Stop Counting Calories   Counting calories is outdated information. It came out before we even understood our bodies and food. If you eat excess calories, the body will gain weight. However, it’s outdated. What we eat matters so much more. It will affect how we gain fat and muscle. In the dancing world, many people have eating disorders. Dancers will starve their bodies and also put poison in it. The body is designed to run with the right fuel. Calories keep people from understanding about serving our bodies with the right type of fuels. People eat fake food for the sake of their calorie allotment. If your food doesn’t have calories, then it’s not food.   [33:30] About Ashleigh’s Programs Ashleigh has been studying the brain for the past three years. She has two different coaching programs. One is focused on people suffering from chronic illnesses, and the other is focused on people who struggle with anxiety and self-doubt. He went from 170 lbs to 130 lbs and had a chronic gut infection. Dr. Jockers started to listen to his body and only ate when he was hungry. At this point, he started playing with intermittent fasting. Jockers developed skin cancer. It was a wake-up call to create balance in his life. After doing some research, Dr. Jockers found the ketogenic diet. Eventually, his skin cancer went away after implementing the ketogenic diet and intermittent fasting.   [07:00] The Benefits of Fasting Jockers is trying to maintain his weight and muscle mass. It is easier for him to maintain his muscle mass with fasting. Dr. Jockers likes to fast for brain function – it allows him to think sharply and quickly. Fasting will also help you feel stronger when lifting. Plus, there are mental and emotional benefits of fasting. Jockers doesn’t eat while traveling – there is no need for it. It’s hard for Dr. Jockers to fast through lunch. He fasts through dinner quite often. Having a break during the day is a ritual for most of us. We are addicted to our rituals, and our rituals revolve around food. “The quality of our lives is all about building resiliency.” -Dr. Jockers   [10:50] Fasting Fights Cancer If we are continually stimulating insulin in our body, then we are unable to heal. All of our cells need repair; they are damaged by stress, so we need a regular repair process. We need to take our damaged cells and repair them. It is more energy efficient to repair cells, then replace cells. Cancer is driven by the constant signal to grow. When we remove the signal to grow, then the body can repair. People are worried that fasting will remove muscle. However, the body will get the signal to stay strong and fast. Fasting will result in an increase in strength. Once you get going to the gym, you will feel just as strong if not stronger. Training is the number one stimulus for muscle growth.   [15:50] Does Glucose or Fat Produce More Energy? Glucose is the fastest process. Burning fat is going to take a lot longer. Burning ketones and fatty acids will produce more energy with fewer waste. If we have carbs, we will want to burn those fast. Being metabolically flexible is the ability to switch back and forth. Without food, our body can burn fat and ketones for fuel.   [19:15] The Best Foods to Eat on a Keto Diet Diets need to be customized. People do the best with grass-fed meats, organic eggs, avocados, olives, olive oils, green vegetables, dark chocolate, nuts, seeds, lemons, and limes. You can get most of the benefits of fruits from lemons and limes. Plus, the lemons and limes do not have sugar. Sour foods are good for stimulating bile and stomach acid. Most people have no idea how to stimulate these things. If you are metabolically flexible, then berries are a great source. Consuming too many berries will not allow your body to go into ketosis. We are not getting enough fat-soluble vitamin A. You can find in egg yolk, grass-fed butter, and organ meat. We should do things that stimulate our stomach acid and bile. Bile will sterilize the small intestine.   [26:50] The Biggest Mistake on the Ketogenic Diet Not including enough herbs is the biggest mistake on the ketogenic diet. Often, people are eating too many dairy products and not enough herbs. Some people feel better with less vegetables. However, they will still do well with ginger tea and herbs with their meats.   [28:00] What to do When You Are Plateauing Find out why you are plateauing. Are you actually getting into ketosis? When you are in ketosis, you should not have cravings. If you are not feeling energetic in ketosis, you may have a thyroid issue. Are the outer-third of your eyebrows thinning? This is a common sign of thyroid problems. Test your fasting insulin. You want to see it between two and five. Check for food sensitivity – you may have a sensitivity to coffee, eggs, or dairy. You can also do your own testing by putting something on your tongue and measuring your pulse. Some people need more stress on their system. Intermittent fasting will ramp up fat burning. Exercise will also help. You can also stress your body with temperature changes using cold showers and saunas.   [38:30] About Keto Metabolic Breakthrough It’s a functional medicine approach to the ketogenic diet.
