His cellmate had been transferred out shortly before his death, leaving Epstein alone despite prior concerns about self-harm. The combination of staffing shortages, ignored protocols, missing or nonfunctional surveillance, and a pattern of administrative negligence created a perfect storm that has fueled widespread skepticism about the official narrative.
