Bipolar Depression Lies to You. CBT Checks the Evidence

Bipolar Depression Lies to You. CBT Checks the Evidence

CBT for bipolar depression, in plain English: the spiral runs on a predictable lie, and there are two places you can cut it before it pulls you all the way down.

A thought record that checks the story for accuracy, and behavioral activation that puts action before the mood catches up.

Bipolar depression doesn't just make you feel bad — it lies to you, and it tells the same lie every time: "You're worthless. This will never lift. Don't bother." Because those thoughts arrive with total conviction, they feel less like feelings and more like facts. This video maps the spiral — mood, thought, withdrawal, worse mood — and shows the two places CBT cuts it. First, the thought record: not "think positive," but think accurate. You cross-examine the heavy thought (evidence for, evidence against, what you'd tell a friend) and write a truer, more balanced verdict — checking the receipt before you pay the bill. Second, behavioral activation, the counterintuitive one: the doing comes first and the feeling follows, so you schedule tiny actions on purpose when you least feel like it, going absurdly small (wash one mug, stand outside two minutes). The honest frame stays front and center: CBT works with medication, it does not replace it in a moderate or severe depression, and it helps most when you learn it early — ideally between episodes, not for the first time at the bottom of a deep one.

▸ In this video:

• The predictable lie bipolar depression tells

• The spiral: mood → thought → withdrawal → worse mood

• Cut 1 — the thought record ("accurate," not "positive")

• A worked example: putting a thought on trial

• Cut 2 — behavioral activation ("action first, mood catches up")

• Go absurdly small — and the honest limits of CBT
