Today, I am lucky to have here with me, a specialist in Weight Management and Bariatric Surgery for adults and adolescents, Dr. Robert Cywes. He has been doing bariatric surgery for 18 years, performing over 8,000 surgeries. This is an episode that originally aired on March 2020, and it was so good we decided to re-release it here today. Dr. Cywes’ medical training began in Cape Town, South Africa, where he received his medical degree from The University of Cape Town. In 1989, Dr. Cywes moved to North America and completed a year-long residency in Pediatric Surgery at Ohio State University’s Columbus Children’s Hospital. After completing his pediatric surgery fellowship at the University of Michigan’s C.S. Mott Children’s Hospital, Dr. Cywes was appointed as an Assistant Professor of Pediatric and Fetal Surgery at Vanderbilt University in Nashville, Tennessee where he did hepatic stem cell research. During this time, Dr. Cywes became increasingly interested in adolescent obesity and the impact of the liver and metabolic syndrome on young patients. Dr. Cywes’ research led to a comprehensive understanding of the toxicity of chronic excessive carbohydrate consumption as the primary cause of obesity and so-called obesity-related co-morbidities, and he became interested in developing a clinical program to treat obese patients using this knowledge. Dr. Cywes relocated to Jacksonville, Florida where he joined the Department of Pediatric Surgery at the Nemours Children’s Clinic and Wolfson Children’s Hospital. This led to a national meeting in Jacksonville where guidelines for adolescent obesity surgery were established. Dr. Cywes established JSAPA to continue his work in both adolescent and adult obesity treatment and surgery, and in 2013 opened a practice in Palm Beach County, Florida. He now works with a highly experienced team of professionals from a variety of medical sub-specialties to better care for obese patients. He has developed the practice into an internationally recognized Center of Excellence for obesity surgery. The practice uses a cognitive behavioral therapy approach that addresses carbohydrate addiction, along with bariatric surgery, to help patients manage their obesity long term. Addicts are usually not able to see how bad it is within themselves. He moved to Canada, where he completed his general surgery residency and specialized in Minimally Invasive Surgery at the University of Toronto. Also, Dr. Cywes earned a Ph.D. in Liver Carbohydrate Metabolism and the effect of glucose metabolism on liver transplant immunology. Cywes learned that the body doesn’t need to run on sugar. In fact, it prefers a keto method of running. He also learned that fatty liver doesn’t come from the consumption of fat. It happens from infusing insulin and sugar. [10:00] About LDL LDL is a complex molecule. It’s a truck on the road transporting fat on your body. LDL will lay down fat over an injury. HDL will come along and remove the fat. LDL is not causing the injury; it’s desperately trying to heal the injury. Think of the liver like a subdivision. You have all the houses which are the different cells. Tiny roads that go to each house. Think about a bunch of garbage that blocks the road. You can’t get food and oxygen downstream to those cells. Eventually, those cells will die. As you infuse sugar into the liver, you will see that swelling. [14:15] The Difference Between Ketosis and Diabetic Ketoacidosis The difference between ketosis and diabetic ketoacidosis is 5.5 – 6 grams of sugar. It’s half to one gram difference. It’s an incredibly small difference. An apple has 25 grams of sugar in it. That’s a five-fold increase in blood sugar. Think about how resilient the body is to take sugar out of the bloodstream. [16:15] Flaws With Conventional Approaches to Diabetes Nothing fixes diabetes. Drugs just slow down the process. Once someone goes on the drugs, they are told to eat sugar. There is a misconception that the body has to have sugar. If you don’t have carbs going in, then you are not going to have the type 2 effect. If you manage your blood sugar tightly, you don’t have insulin resistance. Chronic carbohydrate consumption will cause resistance. If the body protects itself by resistance, then it doesn’t matter what the source of the insulin is. A1C is an injury to blood cells because they are damaged. [20:35] Effectively Treating Diabetes If we are going to treat diabetes effectively, we have to understand how our body protects itself. We can treat diabetes without sugar. We are relying on people who are dependent on the diabetic manufacturing industry to tell us not to eat sugar. It’s counter-productive. How do you tell people that statins are unnecessary? No one can get their hands on an inexpensive CGM. You cannot manage a type 2 diabetic without a CGM. Yet, because of the FDA, these things are so expensive that people cannot afford them. CGMs safe lives. [23:40] The Best Feedback Instrument to Lose Weight The best instrument to lose weight is a CGM. The worst device to lose weight is a scale. You can eat a tub of ice cream, and the scale will go down. You can do a 24 hour fast, and the scale will go up. As soon as you put the first spoon of ice cream in your mouth, your CGM will be alarming. [26:30] Our Bodies Have Feedback Controls for Survival For every action, there is an equal and opposite reaction. If something is not necessary for human survival, then it doesn’t have a feedback system. The systems are very complex at the interface level. The human body will use one molecule to create a whole variety of downstream effects. It is ridiculous to tell people to drink water. Your body knows when it needs water - it will rarely allow yourself to get thirsty. There is no need for a mathematical formula to tell us when to drink. Alcohol is not necessary for survival. There is no stopping point for our bodies. That is why we have a mathematical formula for when to stop drinking. The human body has no need to know when you should stop eating. There is no feedback control for the consumption of carbohydrates. People are not fat loading their food, they are fat overloading. It’s not okay to eat a lump of butter with steak attached. Overeating fat will make you feel like crap afterward. Part of keto is allowing your body to tell you when it has had enough. [40:30] Keto is a Metabolic State There is so much bullshit in nutrition science. The best place to go to is understanding the complexity of human biology. Human beings changed to become better with our brains. To develop brain function, we needed a more significant source of animal fat. We hunted and primarily lived on coastlines with easy access to water and seafood. Our brains need animal fat. Human beings evolved to become omnivores. Our primary focus is on the carnivore diet when we don’t have access to animal fat. If you have to supplement your food with pills, then you are not on a complete diet. [48:30] Dopamine Fasting and Serotonin Loading Carbohydrates are a drug and not food. When you get rid of carbohydrates, they will leave deficits. People don’t understand that the main reason they overuse carbohydrates is because they have a dysfunctional relationship with a substance that gives them instant gratification. If you have a rough day and eat a tub of ice cream; then you’ve had instant gratification. Going on a walk requires effort. However, it allows you to process tough issues that drive emotions. A healthy person can focus for 20 minutes, then they give themselves a serotonin break like a sip of coffee or a deep breath. We have lost track of effective serotonin relief. Go for a walk, do something charitable, do something creative. AND MUCH MORE! He then decided that being a physician was more fitted to his style. After years in the university, he figured that he was also invested in doing research. As a physician, he can help people and do some research at the same time. In choosing a specialty, Dr. Fung felt that Nephrology suited his personality better. Nephrology is a complex specialty which is sort of a thinking-person speciality unlike cardiology which is more of an action specialty (i.e surgery)   [4:13] Diabetes, Obesity and Intermittent Fasting The most common and important cause of kidney disease is Type II Diabetes which is closely related to weight gain and obesity. Treating kidney diseases with medications is the least effective way to treat people. You have to look at the root cause of the disease in order to be successful in treating it. Which is why, for Dr. Fung, the real question was “what causes weight gain?” Nutrition is not considered part of the medical specialty which explains why doctors depend on medications to treat certain illnesses. Second question: What are you gonna do about it? Obesity was not existent during the 1970s. Now, 70% of Americans are obese. Intermittent Fasting: People suffering with Type II Diabetes for 20+ years immediately felt better by intermittent fasting. Fasting is less fun that eating pizza and doughnuts. Fasting is tough work. It is important to give people undergoing intermittent fasting the support and education they need.   [12:30] The Calorie Debacle Coca-Cola created a group called the Global Energy Balance Network to keep people focused on calories. They wanted people to think that 100 calories of broccoli is equivalent to 100 calories of Coca-Cola which is completely wrong. If you think that if you can eat cookies, drink coke and exercise then, you’ll be healthy, you are wrong. But, Coca-Cola wants you to think that this is right. Hormones control hunger, telling our body when to eat and when to stop. Different calorie sources elicit different hormonal responses. Diet and exercise are totally different things. Diet is what you eat. It affects the liver mostly. Exercise is mostly about skeletal muscles. Calorie is not a physiologic term. There’s no calorie-receptors in our body. We think that excessive intake of calories causes obesity. We think that this is a caloric imbalance. Too many ‘calories in’ compared to too few ‘calories out’ leads to weight gain. This Calorie Balance model of obesity has been drilled into us since childhood. Read more about the Calorie Debacle here.     [18:40] Intermittent Fasting as a reasonable alternative to treat patients Ketogenic diet and intermittent fasting are not as famous as low-carb diet. Jason Fung received a lot of backlash when he started to introduce intermittent fasting to his patients. Because of IF, patients were getting better. At the end of the day, you can’t ignore that. Other doctors started to notice the success of IF. Through his book, The Obesity Code: Unlocking the Secrets of Weight Loss, Dr. Fung explains how to use intermittent fasting to break the cycle of insulin resistance and reach a healthy weight—for good.   [25:55] Why the common treatments for type 2 diabetes seem to make things worse Type II Diabetes is a reversible disease. The standard treatment of Type II diabetes focused on getting the sugar down by medications. Doctors should’ve focused on the patient’s diet to help them lose weight because if you lose weight, your diabetes gets better. Jason Fung forever changed the way we think about obesity and he wanted to do the same for type 2 diabetes through his book The Diabetes Code: Prevent and Reverse Type 2 Diabetes Naturally He explains why conventional treatments that rely on insulin or other blood-glucose-lowering drugs can actually exacerbate the problem, leading to significant weight gain and even heart disease. People cannot just make a decision to burn more fat calories, or to feel less hungry. These are physiological issues and must be addressed via physiology.   [31:49] Dr. Fung’s Intensive Dietary Management (IDM) program The IDM program helps people reach their health goals through intermittent fasting and simple-to-follow time-restricted eating strategies. It’s a structured digital lifestyle intervention developed by Megan Ramos and Dr. Jason Fung that provides you with all the education and support you need to succeed. Today’s episode might be a little bit different than all of the previous ones. Still, it is as insightful as ever, especially because I am joined by the mastermind behind the Keto Kamp movement. I am humbled to be joined by Sean Croxton, a podcast host, author, entrepreneur, and prosperity teacher. I followed his growth and career for the past 8 years, most especially when he still hosts the Underground Wellness Radio, one of the internet’s most popular health podcasts.   During his seven years with Underground Wellness, Sean penned the still-popular Dark Side of Fat Loss ebook, started the JERF (Just Eat Real Food) movement, and hosted seven online summit events, including The Thyroid Sessions, Digestion Sessions, and The Depression Sessions.   Now, Sean is the host of The Quote of The Day Show, the #1 daily motivational podcast, featuring short inspirational teachings from the best of the best speakers. He is also the creator of Money Mind Academy, an 8-week course that will completely change your relationship with money.   In this episode, Sean shares the story of how his hardwork and determination got him to where he is today. He also explained the science behind developing a habit while we dive deep into physiology, neurocardiology, brain chemistry and how it is connected with your keto diet and fasting. Tune in! In this episode, you'll discover:   [00:55] Sean Croxton, the early years Sean’s mother worked three jobs. By the time he wakes up in the morning, his mom is already at work. When he goes to bed at night, his mom is still at work.  “If I didn’t have that experience of living at the poverty line, I probably wouldn’t have the value that I have on abundance.” His early years was a great learning experience for him. It showed him the value of earning money.  It showed him the freedom of earning money. It showed him that in order to have more money, you gotta do more service.   [8:18] The audio clip that helped changed Sean’s life  In November of 2009, his friend, Antonio, sent him an audio clip of John Demartini. “The amount of money that you have will always be in direct proportion to the quality and quantity of the service you provide.”   [17:10] After graduating from college, Sean became a personal trainer. His uncle and his aunt were his biggest influences on why he took health in college.  As a personal trainer, Sean had to build up the courage to talk to people when suffering with social anxiety. He had his first panic attack while doing some personal training. His doctor prescribed him Prozac but what he really wanted was Paxil. While training his last client, he found himself in the midst of a “Prozac-induced” panic attack. His social anxiety isn't completely gone and Sean's ok with that. He prefers to call it introversion. [22:32] Back in 2008, Sean launched his first show, Underground Wellness Radio. The podcast ran for 7 years and he released 348 episodes over this period of time. Surprising to most, in 2015 Sean closed up shop with his brand Underground Wellness. “Passions have seasons, and all seasons eventually come to an end.” - Lisa Nichols Sean felt that his passion for interviewing health experts is over.    [26:08] The importance of passion and loving what you do for a living If you are not living according to your highest values, it means you are not really living.  You are living by social idealisms. You are living by something outside of you. Life is too short for that. There’s a little brain inside our hearts that stores information, memories, personalities, etc. Our hearts are our point of attraction.  Heart rate variability is the physiological phenomenon of variation in the time interval between heartbeats. We don’t want these to be to low. Sleep deprivation is such a chronic condition these days that you might not even realize you suffer from it. Science has now established that a sleep deficit can have serious effects on your health.   For example, interrupted or impaired sleep can: Dramatically weaken your immune system Accelerate tumor growth—tumors grow two to three times faster in laboratory animals with severe sleep dysfunctions Cause a pre-diabetic state, making you feel hungry even if you’ve already eaten, which can wreak havoc on your weight Seriously impair your memory; even a single night of poor sleep—meaning sleeping only 4 to 6 hours—can impact your ability to think clearly the next day Impair your performance on physical or mental tasks, and decrease your problem solving ability 06:12 Most fat burning takes place during stage 4 sleep.
