The morning Jeffrey Epstein was found dead in his Metropolitan Correctional Center cell, nearly every major jail protocol was broken. He was supposed to be checked every 30 minutes under suicide-watch procedures, yet the guards on duty failed to make their rounds for hours. His cellmate had been transferred out the night before and was never replaced, directly violating Bureau of Prisons policy that required Epstein to never be left alone.
