The Painful Stiff Big Toe: Understanding Hallux Rigidus

The Painful Stiff Big Toe: Understanding Hallux Rigidus

A stiff, painful big toe may not seem like a serious problem—until every step becomes uncomfortable. In this episode, we explore hallux rigidus, also known as big toe arthritis, a progressive condition that causes pain, stiffness, and loss of motion in the joint at the base of the big toe. We explain why this arthritis develops, how it gradually changes the way you walk, and why activities such as walking, running, climbing hills, or even wearing certain shoes can make symptoms worse.

Using clear, patient-friendly language, we discuss the common signs of hallux rigidus, including swelling, bone spur formation, and limited motion of the big toe. You'll learn how doctors diagnose the condition using a physical examination and weight-bearing X-rays, and why many patients begin to shift their weight to the outside of the foot, sometimes leading to pain in other areas of the foot.

Finally, we review the full range of treatment options. We explain how stiff-soled shoes, orthotics, anti-inflammatory medications, and activity modification can often provide significant relief. For patients with persistent symptoms, we discuss the surgical options—including bone spur removal (cheilectomy), fusion of the big toe joint, and joint replacement—and explain the advantages and limitations of each approach. Whether you're beginning to notice stiffness in your big toe or considering surgery, this episode will help you better understand your condition and the treatments available.
