EP. 36 The "July Effect" The Incoming of New Residents

This week's episode I discuss the "July Effect" this phenomenon that occurs every July where we see a new group of residents and the high volume of ordering CTs mostly in the emergency departments.

This can be frustrating for patients and CT departments as patients can receive unnecessary exams, overexposure due to additional exams ordered requiring multiple trips to the CT department.

Communication is very important between the residents and the technologists for the sake of the patient.

5 ways we can help residents utilize CT appropriately:

1. Review of patient history and prior imaging: this can help in unnecessary CT exams. This can be challenging when patients are poor historians, unconscious, trauma, etc.

2. Communication with Radiologists: this is important as it takes the technologist out of playing the middle man. Radiologists can provide input on the correct imaging based on the patient's clinical history.

3. Use of Alternative Imaging Modalities: The use of ultrasound or MRI can provide better imaging based on the patient's clinical history if appropriate.

4. Protocol-Driven Methods: Having clear protocols and guidelines for different clinical scenarios have help residents order accurate CT exams.

5. Decision Support Tools: encourage the use of Image Wisely imaging exams criteria tool that can give them guidance on best imaging practices.
