It will cause you to burn excess body fat and improve your overall health. But there’s one important caveat. A constant, unrelenting state of ketosis—in which your body is burning its own fat for fuel—is obviously unsustainable, because eventually you’d have no more fat. In this chapter, we move into the Flex pillar, which will allow you to maintain your Keto lifestyle month after month, year after year. Four Reasons Why Prolonged Ketosis Is Counterproductive My mantra is, “Lose the excess fat; treasure the necessary fat.” Ketosis is a brilliant strategy for accomplishing the first goal. As for the second goal, prolonged ketosis doesn’t have an automatic shut-off valve. If you remain in state of constant ketosis, burning your own fat, then your body will keep doing that as long as it must to stay alive. If necessary to preserve life, once your fat is gone, various bodily functions will be shut off or minimized. This process of shutting down will continue until Mr. Death knocks on your door and says, “Sorry, buddy—time’s up.” No one wants that. To ensure that you’re well informed, let’s review some of the bad things that can happen if you go too far with ketosis. Fat Burning Slows Down As levels of body fat drop, your primal brain, whom we thought we had trained to accept a high-fat diet and fasting, will be aroused and will begin to object. Why and when will this happen? Your primal brain, that eternal watchdog, will sense that your reserves of fat are becoming depleted. Remembering those long periods of deprivation thousands of years ago, your primal brain will sound the alarm: The fat supply is running low! It will issue the command that your body must preserve its precious fuel. In response, your body will actually slow down fat burning—all for the sake of survival! When you stay in ketosis too long, the body starts to rebel, and conserve its fuel. A good analogy is if you lived out in the woods and you had stored 500 logs of firewood to heat your cabin for the winter. These were your reserves, because you could still go out and chop more firewood. So if you had to dip into these reserves, you wouldn’t be worried. But now it’s early March, and snow still covers the ground. It’s been a tough winter and you haven’t harvested many trees, and you see that you’ve got only 50 logs left. That’s a very low supply! Suddenly you’re reluctant to use them because you don’t want to run out. When you had 500 logs, you didn’t care about burning them. The same thing happens with your body fat. This is why when you stay low carbohydrate for too long, your body will slow down fat metabolism. And you know what? This is a good thing. There is a big difference between losing excess fat and losing all your fat. You want to lose excess fat. But your body needs a small amount of fat, and your primal brain is smart to insist that you meet this minimal requirement. Thyroid Problems Hormone production is often a function of a “chain of command,” as orders are passed from one organ to another, until the desired outcome is achieved. The hypothalamus and the pituitary gland, which are located in the brain, help control the thyroid gland, a butterfly-shaped gland located at the base of your throat, just below the Adam’s apple. To provide their direction to the thyroid, the hypothalamus releases thyrotropin-releasing hormone (TRH), which stimulates the pituitary gland to release thyroid-stimulating hormone (TSH). When the hypothalamus and pituitary are working normally, they respond when: Thyroid hormone levels are low, so they secrete more TRH and TSH, which stimulates the thyroid to make more hormones. Thyroid hormone levels are too high, so they secrete less TRH and TSH, which reduces hormone production by the thyroid. The thyroid gland uses iodine from food to make two thyroid hormones: triiodothyronine (T3) and thyroxine (T4). The primary secretory product is inactive thyroxine (T4), which is a prohormone of triiodothyronine (T3). T4 is converted to T3 peripherally by tissues with high blood flow, including the liver and kidneys. In the brain, T4 is converted to active T3 by type 2 deiodinase produced by glial cells. The thyroid stores T3 and T4, and releases them as they are needed. These two thyroid hormones function in almost every cell in your body, and play a fundamental role in regulating your metabolism and maintaining your resting energy expenditure—that is, the energy you burn just to keep functioning while sitting on the sofa. Diet can influence thyroid hormone levels, and prolonged ketosis and severe calorie restriction can lead to reduced thyroid function, or hypothyroidism, which is a mechanism to ensure survival in response to perceived famine. The goal is to slow metabolism, preserve energy, and preserve vital organs. T4 is the inactive form of thyroid, it needs to be converted to T3 which is the active form used by the cells. Do you know which hormone helps with this conversion? Insulin. Low thyroid hormones cause a drop in levels of insulin, the hormone needed to transport sugar from the blood to different cells throughout the body. Chronic low insulin levels slow down cellular function, including muscle contractions and basic brain function. Over longer periods of time, low insulin levels compromise this T4 to T3 thyroid hormone conversion. Testosterone Deficiency Sex hormone binding globulin (SHBG) is a protein made by your liver. It binds tightly to three sex hormones found in both men and women—estrogen, dihydrotestosterone (DHT), and testosterone. SHBG carries these three hormones throughout your blood. SHBG controls the amount of testosterone that your body tissues can use. Too little testosterone in men and too much testosterone in women can cause problems. Factors such as sex, age, obesity, liver disease, and hypothyroidism can change the level of SHBG in your blood. Hypothyroidism is when your thyroid produces an insufficient amount of thyroid hormones, which can happen with prolonged ketosis. What I’ve seen reviewing lab work over the years is too much SHBG leads to lower testosterone levels, and not enough SHBG leads to an increase in estrogen; both can be problematic for men and women. Long term ketosis can lead to either of these. Leptin Reset Leptin is your “stop eating!” hormone that plays an instrumental role in your hunger and weight management. It comes from the Greek word ”leptos,” meaning “thin.” Fat cells produce leptin in proportion to levels of body fat: the more fat cells you have, the more leptin they make. When your body fat increases, your leptin levels go up. Your brain senses the increase in leptin, and thinks, “Okay, we’ve got enough fat in reserve, so we can stop eating.” Thus, you eat less and burn more. Conversely, when you don’t eat, your body fat decreases, leading your leptin levels to drop. Your brain, sensing a shortage of leptin, thinks, “Hmm, we’re low on fat reserves. Better find some food!” At that point, you eat more and burn less. This negative feedback loop is similar to the control mechanisms for many physiological functions, such as body temperature, breathing, and blood pressure. A properly working leptin system tells your brain that you have enough energy stored in your fat cells to engage in normal, everyday metabolic processes. It leads to better brain function, memory, metabolic performance, mental sharpness, and coordination, and it can even affect the regulation of mood and emotion. This is wonderful—except that many overweight people suffer from leptin resistance. They have plenty of blood leptin, but the brain doesn’t recognize it. Despite the abundance of leptin, the brain thinks there is too little leptin—and orders the body to keep eating! This is obviously a bad situation. What can cause it? Several potential mechanisms behind leptin resistance have been identified. Free fatty acids. Having elevated free fatty acids in your bloodstream may increase fat metabolites in your brain, and interfere with leptin signaling. Inflammation. Inflammatory signaling in your hypothalamus could be a cause of leptin resistance. Having consistently high leptin. It may be that your brain gets accustomed to a constant flood of leptin churned out by your fat cells, and becomes “numb” to it. So, what happens when you follow a low-carb keto diet that puts you in long-term ketosis? Perhaps paradoxically, many people do not suffer from an increase in appetite, as you might expect from falling levels of leptin. It’s complicated, but the short version of the story is that leptin levels will drop, which would normally signal your brain to start eating. But something else happens—your brain becomes more sensitive to the effects of leptin, so a smaller amount of leptin packs more punch. In addition, the brain’s response to leptin is inhibited by inflammation, resulting in increased leptin resistance. Obesity increases inflammation, which increase the brain’s “numbness” to high levels of leptin. Inflammation is dramatically reduced by sustained nutritional ketosis. The reduction in leptin resistance due to reduced inflammation adds to the brain’s heightened sensitivity to leptin. In other words, on a well-formulated ketogenic diet the brain perceives a greater satiety response to less leptin. This explains the paradox as to why we see a decrease in appetite with nutritional ketosis. In order to achieve a sustained state of ketosis, you need to occasionally boost your levels of leptin and send the signal to your brain that higher levels of leptin are normal, and that you need to keep eating. Robinson J. joined a harvest party with a Sicilian family in Italy. The moment T.J. tasted fresh olive oil, it changed his whole life. Fresh olive oil could change the world, people’s diet, and their health.   [04:30] The Difference Between T.J.’s Olive Oil and Mass-Produced Olive Oil Bulk oil is made up of oils from different countries. When you look at the label, it may have five or six different countries on it. There is very little care with these oils. Some bulk oils can sit in vats for years and years before ever making it to your pantry. J. works with small family farmers who do not have access to the United States market. It is a highly curated collection, following a seasonal harvest. After trying hundreds of oils, T.J. makes test batches and eventually sends the oils out to his club members. The health qualities cannot be compared between T.J.’s olive oil and bulk olive oils.   [08:20] About the Olive Harvest There are over five hundred different olive varieties in Italy. After playing with the varieties, T.J. creates blends. The ripeness of the olives will alter the flavor profile. As the fruit goes from green to black, more oil is created inside the fruit. If the farmer is producing oil for bulk oil, they will leave it on the tree for an extra few months. During this time, it goes from 10% oil to 30% oil. The health-promoting qualities of olive oil are the greatest when it’s super green and not as ripe. Harvesting the fruit when it’s green is not as cost-effective as harvesting it when it’s overripe. Bulk producers harvest when it’s overripe because it produces more oil.   [17:30] Staying Satiated   Olive oil is one of the best fats for staying satiated. Ben will take a shot of olive oil as a crutch when fasting. J.’s olive tastes so good that you can drink it out of a shot glass. The media is finally picking up on the good fats in olive oil. They realize the younger olive oils are better.   [21:00] The Myth About Cooking With Olive Oil Myth: you shouldn’t cook with olive oil. The antioxidants in fresh oil protect it from oxidizing, whether it’s in the body or the frying pan. J. heats his pan first, at the very last minute, T.J. will add his oil. Do not let the oil sit and burn.   [26:40] Taste-Testing Olive Oils Professional taste-testers drink olive oil out of white cups. The color is an indicator of quality. Quality olive oils will have a green tint. Then, swirl the oil and put the cup in the palm of your hand to warm the oil. The first evaluation of the oil will be on the nose. When it is extra-virgin, the oil goes through a professional tasting panel and a chemistry panel. Take a whiff of the oil. You should notice that it smells like being in a vegetable garden. After you have smelled the oil, it is time to place a teaspoon of oil on your palette and chew the oil. T.J. tries to draw a little air in when he tastes. During this, you are looking for bitterness and spiciness. You should feel a tingle in your throat.   [35:20] Uses for Olive Oil Ben takes a shot of olive oil to stay satisfied. He also uses it for eggs, salad dressings, and sautéing vegetables J. likes to use it as a finishing oil. It would be best if you used it as a sauce and can use it to roast vegetables. Today I interview Dr. Paul Saladino, the leading authority on the science and application of the carnivore diet. He has used this diet to reverse autoimmunity, chronic inflammation, and mental health issues in hundreds of patients, many of whom had been told their conditions were untreatable.   In addition to his podcast, Fundamental Health, he can be found featured on numerous podcasts, including The Minimalists, The Model Health Show, Bulletproof Radio, The Dr. Gundry Podcast, The Ben Greenfield Podcast, Dr. Mercola, Health Theory, Mark Bell’s Power Project, and many others. He has also appeared on The Doctors TV show and will release his first book in 2020 titled, “The Carnivore Code: Unlocking the Secrets to Optimal Health by Returning to our Ancestral Diet.”   Dr. Paul Saladino is board certified in psychiatry and completed a residency at the University of Washington. He also is a certified functional medicine practitioner through the Institute for Functional Medicine. He attended medical school at the University of Arizona, where he worked with Dr. Andrew Weil focusing on integrative medicine and nutritional biochemistry.   Prior to medical school, Dr. Paul worked as a physician assistant in cardiology. Dr. Paul defines the carnivore diet. The diet cuts out all plants and is entirely animal-based. By removing plants from your diet, plant toxins will also get cut out. Dr. Paul reveals which plants should be removed from your diet because of toxicity. Stop eating seeds, leafy greens, and sweet fruits to avoid nasty toxins. When he got sick, he also wanted to be able to fix his health. “We are part of this bodily machine. I want my machine to be as tuned as can be.” – Dr. Paul Saladino Paul didn’t want to go to the doctor to find out that he needs medicine, and just have to believe that. He wants to be the agent of his health and his family’s health.   [13:30] About the Carnivore Diet It’s a diet that is entirely based on animal foods and excludes all plant foods. There’s a version of the diet that does include some plant foods. Anything you can hunt is going to be animal food. Carnivore diets exclude plant foods and appreciate the spectrum of plant toxicity. Paul gives people the tools to make the right choice for them. When Dr. Paul made the transition from paleo to carnivore, he cut out all the plant foods. There are toxins in plants that he completely eliminated. Plant toxins can be damaging to the gut. Leaky gut is gaining more widespread acceptance.   [21:00] The Practice of Fasting Paul is at the weight he wants to be at. He does not use fasting as a tool. Fasting can be used as a valuable tool. We need to be careful about how we use it. For some people, fasting may be too much.   [22:30] The Carnivore Code The book is entirely written with 425 references. Paul wants the book to be readable, yet he wants to include all the science. You can preorder The Carnivore Code here. Paul’s favorite thing about the book is the anthropology. The first part of the book is all about humans.   [33:10] Plant Toxicity Spectrum   Plants and animals have been coevolving. Plants have always been absorbing toxins. The seeds of plants are most vulnerable. Grains, seeds, beans, and nuts are all seeds. If a plant is putting energy into making a reproductive part, they are going to put the most toxins in the seed to prevent them from consumption. If a plant is full grown and you eat one or two leaves, it won’t kill the plant. Eating the seeds will kill the future of the plants. Paul says not to eat all the nuts as most keto followers do. Other high toxic plants are spinach, rhubarb, beets, and almonds. Eliminate all seeds, leafy greens, and sweet fruits.   [46:15] Living a Radical Life A radical lifestyle includes occasional fasting, low-carb diet, exercise, sun, cold, and heat exposure. These activities will result in optimal antioxidant status and longevity genes. We do not need plant molecules for longevity genes. We can take anti-inflammatory medicine; however, there are long-term effects of medicines. We do not need many plant molecules because there are adverse side effects, just like medicine.   [50:00] Side Effects to Eating Animal Meat Animals do not make defense compounds like plants do. Animals have the ability to defend themselves. The majority of animals do not have toxins in their muscles. The cooking of foods will create toxins for humans. The lower the temperature you cook, it will lessen the cooking toxins. The higher the temperature and the faster we cook, the higher the chance of toxins. Grain-fed meat is going to be higher in toxins. Today, I have here with me, Mike Mutzel, YouTuber, podcaster, and teacher of leading-edge science that can be utilized by progressive clinicians for the prevention of chronic disease. Often termed Translational Medicine, this bench-to-bedside approach is critical to ensure the application of pioneering research and concepts in systems biology, a preventative health care setting, optimizing patient outcomes.   Mike earned his B.S. in Biology from Western Washington University in 2006 and completed his M.S. in Clinical Nutrition from the University of Bridgeport in 2015 and is a graduate of the Institute for Functional Medicine’s (IFM) Applying Functional Medicine in Clinical Practice (AFMCP).   He is a Functional Medicine consultant in the natural product industry and has worked as a nutritionist with Gerard Guillory, MD of the Care Group, P.C. in Denver, CO. Mike has lectured alongside Dr. Guillory to large patient-groups in the Denver area, and they have been featured on local T.V. programs discussing the power of Nutrition and Functional Medicine.   In April of 2014, Mike published his first book, Belly Fat Effect: The Real Secret About How Your Diet, Intestinal Health, and Gut Bacteria Help You Burn Fat.   In this episode, Mike Mutzel describes how he started in the health world and why he is so attracted to gut health. For the first time in his life, Mike felt depression - he didn’t know what was wrong with him but knew he needed to test his hormones. Mike’s testosterone was extremely low. He started to find out how the food he was eating was affecting his testosterone levels. Overtraining can also affect testosterone. Mike works with doctors to help their patients with diet and lifestyle changes. His YouTube channel launched in 2012. Don’t be afraid to put out YouTube videos; you are going to get better over time.   [11:30] About Our Guts All the crappy foods we eat are high in calories and carbohydrates. They also perturb our microbiome. Microbiome contributes to the inflammatory tone in our bodies. If there is an imbalance, our immune system will also be out of whack. When we eat carbs, and our blood sugar goes up, that is only part of the story. Our brain is connected to our gut. The mechanical exposure to foods affects the different cells. Before we swallow things, our body is anticipating the food. If we train our brain to chew food mindlessly, then that is what will keep happening. We need to slow down, chew our foods, and put down the forks. Fasting will also come into play. We do not need to eat every two to three hours. “A lot of people don’t even know if they are hungry or not anymore.” -Mike Mutzel   [18:15] The Importance of a Diverse Diet A lack of diversity is a lack of stability. People with allergies seem to fair better with a low-carb diet. It’s exactly the opposite of what many people thought five years ago. We cannot totally manipulate our diet to have massive swings in the immune system. Be mindful of ketones, and keep track of your vitals. There are healthy people with low diversity and unhealthy people with high diversity.   [21:45] Seasonal Eating Mike personally emphasizes seasonal eating. He is grateful and fortunate enough to have the financial resources to eat seasonally. Some people do not have the income or resources to eat seasonally. Seasonal eating is an essential piece of the puzzle. Even though blueberries are considered a healthy food; it’s healthy food out of context. Once you get the fast food out of the house and prioritize sleeping, then you can start thinking about eating seasonally.   [33:00] The Importance of Healthy Bile   Bile makes it all the way to the heart and the brain. Bile is so important and underutilized. Mike promotes bile salts. When people are constipated, recirculating bile is not healthy.   [37:10] The Problem with Game Changers The movie is compelling, and loads of people are convinced to go vegan. Mike has tried a vegan diet; it doesn’t accelerate recovery. On a vegan diet, our cells do not regenerate faster. Mike has no problems eating vegetables and fruits. You didn’t see any of the food people ate until the credits. The foods they were eating were a lot of grains.   [44:30] What is the Parasympathetic Tone? Our nervous system has two personalities. Fight or flight mode and relax mode. So many people are hung up in the fight or flight mode. We need to be proactive about unwinding our fight or flight mode. Yoga and meditation are effective and unwinding our fight or flight. There are anti-inflammatory properties when we are in the parasympathetic state. The metabolism thats required to do that generates something called reactive oxygen species (ROS). ROS creates free radicals that are damaging to your cells and your mitochondrial DNA, which are then transferred to your nuclear DNA. Your body ages from the damaging aspects of excessive ROS. Eat before bed = age faster. Your energy consumption drops dramatically when you are sleeping. This tends to expand the production of ROS when you don't use the energy. Higher amounts of ROS = cellular damage. This can damage your tissue, your cell membrane, which is the bodyguard of your cell. Additionally, when your body has to digest food during sleep; it takes energy away from recover repair and fat burning and diverts it to digesting food. It takes massive amounts of resources and blood flow to digest food. The first 4 hours of sleep are the most crucial when it comes to getting the most important stage. This is called money time sleep window. 98% of fat burning takes place during stage 4 sleep. This is why I recommend fasting at least 3 hours before bed.
