
Dr Robert Cywes | Effectively Treating Diabetes, Calories In vs Calories Out BS, & The Problems With Too Much Fat on Keto KKP: 340
Today, I am lucky to have here with me, a specialist in Weight Management and Bariatric Surgery for adults and adolescents, Dr. Robert Cywes. He has been doing bariatric surgery for 18 years, performing over 8,000 surgeries. This is an episode that originally aired on March 2020, and it was so good we decided to re-release it here today. Dr. Cywes’ medical training began in Cape Town, South Africa, where he received his medical degree from The University of Cape Town. In 1989, Dr. Cywes moved to North America and completed a year-long residency in Pediatric Surgery at Ohio State University’s Columbus Children’s Hospital. After completing his pediatric surgery fellowship at the University of Michigan’s C.S. Mott Children’s Hospital, Dr. Cywes was appointed as an Assistant Professor of Pediatric and Fetal Surgery at Vanderbilt University in Nashville, Tennessee where he did hepatic stem cell research. During this time, Dr. Cywes became increasingly interested in adolescent obesity and the impact of the liver and metabolic syndrome on young patients. Dr. Cywes’ research led to a comprehensive understanding of the toxicity of chronic excessive carbohydrate consumption as the primary cause of obesity and so-called obesity-related co-morbidities, and he became interested in developing a clinical program to treat obese patients using this knowledge. Dr. Cywes relocated to Jacksonville, Florida where he joined the Department of Pediatric Surgery at the Nemours Children’s Clinic and Wolfson Children’s Hospital. This led to a national meeting in Jacksonville where guidelines for adolescent obesity surgery were established. Dr. Cywes established JSAPA to continue his work in both adolescent and adult obesity treatment and surgery, and in 2013 opened a practice in Palm Beach County, Florida. He now works with a highly experienced team of professionals from a variety of medical sub-specialties to better care for obese patients. He has developed the practice into an internationally recognized Center of Excellence for obesity surgery. The practice uses a cognitive behavioral therapy approach that addresses carbohydrate addiction, along with bariatric surgery, to help patients manage their obesity long term. Addicts are usually not able to see how bad it is within themselves. He moved to Canada, where he completed his general surgery residency and specialized in Minimally Invasive Surgery at the University of Toronto. Also, Dr. Cywes earned a Ph.D. in Liver Carbohydrate Metabolism and the effect of glucose metabolism on liver transplant immunology. Cywes learned that the body doesn’t need to run on sugar. In fact, it prefers a keto method of running. He also learned that fatty liver doesn’t come from the consumption of fat. It happens from infusing insulin and sugar. [10:00] About LDL LDL is a complex molecule. It’s a truck on the road transporting fat on your body. LDL will lay down fat over an injury. HDL will come along and remove the fat. LDL is not causing the injury; it’s desperately trying to heal the injury. Think of the liver like a subdivision. You have all the houses which are the different cells. Tiny roads that go to each house. Think about a bunch of garbage that blocks the road. You can’t get food and oxygen downstream to those cells. Eventually, those cells will die. As you infuse sugar into the liver, you will see that swelling. [14:15] The Difference Between Ketosis and Diabetic Ketoacidosis The difference between ketosis and diabetic ketoacidosis is 5.5 – 6 grams of sugar. It’s half to one gram difference. It’s an incredibly small difference. An apple has 25 grams of sugar in it. That’s a five-fold increase in blood sugar. Think about how resilient the body is to take sugar out of the bloodstream. [16:15] Flaws With Conventional Approaches to Diabetes Nothing fixes diabetes. Drugs just slow down the process. Once someone goes on the drugs, they are told to eat sugar. There is a misconception that the body has to have sugar. If you don’t have carbs going in, then you are not going to have the type 2 effect. If you manage your blood sugar tightly, you don’t have insulin resistance. Chronic carbohydrate consumption will cause resistance. If the body protects itself by resistance, then it doesn’t matter what the source of the insulin is. A1C is an injury to blood cells because they are damaged. [20:35] Effectively Treating Diabetes If we are going to treat diabetes effectively, we have to understand how our body protects itself. We can treat diabetes without sugar. We are relying on people who are dependent on the diabetic manufacturing industry to tell us not to eat sugar. It’s counter-productive. How do you tell people that statins are unnecessary? No one can get their hands on an inexpensive CGM. You cannot manage a type 2 diabetic without a CGM. Yet, because of the FDA, these things are so expensive that people cannot afford them. CGMs safe lives. [23:40] The Best Feedback Instrument to Lose Weight The best instrument to lose weight is a CGM. The worst device to lose weight is a scale. You can eat a tub of ice cream, and the scale will go down. You can do a 24 hour fast, and the scale will go up. As soon as you put the first spoon of ice cream in your mouth, your CGM will be alarming. [26:30] Our Bodies Have Feedback Controls for Survival For every action, there is an equal and opposite reaction. If something is not necessary for human survival, then it doesn’t have a feedback system. The systems are very complex at the interface level. The human body will use one molecule to create a whole variety of downstream effects. It is ridiculous to tell people to drink water. Your body knows when it needs water - it will rarely allow yourself to get thirsty. There is no need for a mathematical formula to tell us when to drink. Alcohol is not necessary for survival. There is no stopping point for our bodies. That is why we have a mathematical formula for when to stop drinking. The human body has no need to know when you should stop eating. There is no feedback control for the consumption of carbohydrates. People are not fat loading their food, they are fat overloading. It’s not okay to eat a lump of butter with steak attached. Overeating fat will make you feel like crap afterward. Part of keto is allowing your body to tell you when it has had enough. [40:30] Keto is a Metabolic State There is so much bullshit in nutrition science. The best place to go to is understanding the complexity of human biology. Human beings changed to become better with our brains. To develop brain function, we needed a more significant source of animal fat. We hunted and primarily lived on coastlines with easy access to water and seafood. Our brains need animal fat. Human beings evolved to become omnivores. Our primary focus is on the carnivore diet when we don’t have access to animal fat. If you have to supplement your food with pills, then you are not on a complete diet. [48:30] Dopamine Fasting and Serotonin Loading Carbohydrates are a drug and not food. When you get rid of carbohydrates, they will leave deficits. People don’t understand that the main reason they overuse carbohydrates is because they have a dysfunctional relationship with a substance that gives them instant gratification. If you have a rough day and eat a tub of ice cream; then you’ve had instant gratification. Going on a walk requires effort. However, it allows you to process tough issues that drive emotions. A healthy person can focus for 20 minutes, then they give themselves a serotonin break like a sip of coffee or a deep breath. We have lost track of effective serotonin relief. During her research, Nina came across studies pointing out the flaws in a low-fat diet. However, so many scientists at universities were terrified to talk to Nina about fat. Why? Because people in the food industry were paid to give scientists a hard time. Nina realized there was something really wrong going on here that had such enormous implications. Nutrition science from the very beginning has been heavily influenced by the food industry. For instance, the vegetable oil industry wanted to go after people for attacking vegetable oils in any way. You see the same tactics happening today on social media. [08:55] Polyunsaturated Fats vs. Saturated Fats vs. Monounsaturated Fats Fatty acids are long carbon chains. Poly means many. Polyunsaturated fats have many double bonds. Each time there's a double bond in the chain, it kinks the chain. Some examples of polyunsaturated fats: Corn oil Safflower oil Sunflower oil Canola oil Monounsaturated fats are the same carbon chain but with one double bond. If you want to use oil, it’s better to use olive oil because it only has one double bond. Saturated means that the carbon chain is statured with hydrogen, and there is no double bond. It is a flat chain, and it makes a solid like butter. Saturated fats are actually the best for cooking, and they're the healthiest for eating because they do not oxidize or cause inflammation. [17:55] What Went Wrong In The 1950s With Ancel Keys and President Eisenhower The idea that fat, cholesterol, and animal foods are bad for health began in the 1950s. The US was really in a panic over the rising tide of heart disease, which had been virtually nonexistent in the early 1900s. In the 1930s, heart disease became the number one killer. A panic started when President Eisenhower himself had a heart attack in 1955. Eisenhower’s doctor was convinced by Ancel Keys that saturated fats and dietary cholesterol cause heart disease. Plus, Ancel Keys got his ideas adopted by the American Heart Association. Ancel came out with a statement saying to avoid saturated fat and cholesterol in order to prevent heart attacks. We are still dealing with this in 2021. [23:00] Why Are Seed Oils Approved by the American Heart Association? How is it possible for the American Heart Association to continue to ignore so much scientific literature? More than 20 papers published in the field have concluded that we got it wrong on saturated fats. There is no data to justify the authorities' limits on these seed oils. It’s challenging to trust the government with our health when they recommend seed oils. [31:00] Sorry Vegans, Meat-Eating Is Healthy For Humans Nina did not know how controversial red meat was until she put a picture of it on the cover of her book. Sadly, many people think that if you eat meat, you're a bad person, you don't believe in climate change, and you're not doing your part to protect the climate. There's this moral revulsion that has been created against meat. Influencers in the spheres of science and public health have vilified one of the healthiest foods on the planet. Meat was developed by humans in order to have a convenient and cheap source of protein. [33:15] Myth: Meat Causes Diabetes Diabetes is caused by elevated blood sugars. Sugar causes elevated blood sugars, so do carbs. Where's the sugar in meat? It's just nonexistent. It's impossible that meat would cause diabetes. Red meat consumption has gone down around 28% since 1970. Whereas, diabetes has skyrocketed out of control. [34:15] Myth: Red Meat Causes Colorectal Cancer The WHO decided that red meat and processed meats cause colorectal cancer. This decision was based on these kinds of weak science called epidemiological or observational studies that show associations instead of causation. The data they used was self-reported dietary data, and we all lie about what we eat. There needs to be a more significant discussion about the bias against red meat. [38:30] Why People Are Pushing Vegetarian and Vegan Diets The animal rights movement does not want people eating animals. The Seventh Day Adventist Church has a religious belief that people should eat a vegan diet.Plus, the food industry has an enormous investment in plant-based foods. Also, many companies want to hop on the climate change bandwagon. Many vegan burgers have vegetable oils that are not healthy. [41:30] Myth: There’s a Correlation Between Red Meat Consumption and Heart Disease The original reason that red meat was thought to cause heart disease was based the fact that it contains saturated fat and cholesterol. However, research shows that saturated fat and cholesterol do not cause heart disease. Therefore, foods containing saturated fat and cholesterol don't cause heart disease. The correlation between saturated fat, cholesterol, and heart disease has been more rigorously and extensively studied than almost any other scientific question in the history of nutrition science. [47:15] Which To Cut First: Smoking Cigarettes, Processed Sugars, or Vegetable Oils People smoked historically for hundreds of years tobacco, but they did not get cancer or heart disease until the introduction of sugar. Nina says that smoking is probably safer than processed sugars or vegetable oils. Between sugar and vegetable oils, Nina says that vegetable oils are safer. Some spots are also hormonal things like melasma, which you can get from hormonal changes. The number one thing you could do for your skin is actually to protect it from the sun. Getting burnt is not good for your skin. Regular sun exposure will cause DNA damage to the skin. Plus, it can contribute to faster aging. Dr. Killen says to put sunscreen on your face. [07:50] Getting A Healthy Balance of Autophagy and mTOR Through Fasting Even with a few hours of fasting, you can start to see increased stem cell production. He is a Professor of Pediatrics at The Children’s Hospital of Los Angeles, which is affiliated with the Keck School of Medicine at the University of Southern California. He is Program Director for Diabetes and Obesity at The Saban Research Institute and he holds the Dr Robert C and Veronica Atkins Endowed Chair in Childhood Obesity and Diabetes. Dr Goran is a native of Glasgow, Scotland, and received his Ph.D. from the University of Manchester, UK (1986) and has previously served on the faculty of Medicine at the University of Vermont (1991 to 1994), the Department of Nutrition Sciences at UAB (1994 to 1999), and the Department of Preventive Medicine at USC (1999 to 2018) prior to joining Childrens Hospital of Los Angeles. His research has been continuously funded by the National Institutes of Health and other Foundations for the past 30 years and he has published over 350 professional peer-reviewed articles and reviews. He is the Editor of the “Childhood Obesity: Causes, Consequences and Intervention Approaches” published in 2017, co-editor of “Dietary Sugars and Health”published in late 2014, and currently serves as Editor-in-Chief for Pediatric Obesity. He has been the recipient of a number of scientific awards for his research and teaching, including: The Nutrition Society Medal for Research (1996), The Lilly Award for Scientific Achievement from The Obesity Society (2006), The Bar-Or Award for Excellence in Pediatric Obesity Research, from The Obesity Society (2009), the TOPS award for contributions to obesity research from The Obesity Society (2014), and the Rank Prize Lecture in Nutrition (2018). Michael wanted to learn about chronic diseases and how they started in childhood. At the end of the day, he found a lot of very interesting information about sugar and how to modify it. Michael wanted to get that information out there to the public. If someone is diabetic, that didn’t happen over the past year. Instead, they have been eating a poor diet for the last ten to fifteen years. Babies are actually born with a built-in preference for sweetness. It’s supposed to help them seek out good-tasting foods. Food companies know that babies and kids prefer sweetness. So that's why 80% of kids’ foods have some type of added sugars. Breast milk does have carbohydrates. Lactose is built from glucose and galactose. However, there's no fructose in natural mother's milk. It's the introduction of fructose that starts to cause problems as well as added sugar. [07:50] When Moms Should Start To Wean The Baby Off of Breast Milk The recommendation is six months. However, there are studies that demonstrate the longer the duration of breastfeeding, the more likely you are to prevent obesity. Plus, the longer you breastfeed, you will promote brain development. If a mom breastfeeds for more than twelve months, those babies are highly protected from obesity, even if they consume high amounts of sugary beverages as a toddler. [10:55] A Calorie Isn’t Always A Calorie: 90% of Fructose Is Cleared By The Liver Sucrose, which is ordinary table sugar, is what we call a disaccharide. It's a glucose molecule connected to a fructose molecule; both are identical chemically. Glucose is used everywhere in the body for energy. The liver filters out almost 90% of fructose. The liver's job is to filter things out that it doesn't want in the blood. The liver will take fructose and convert it to fat. The most common form of liver disease is nonalcoholic, and that nonalcoholic contributor is fructose. [18:10] Recommendations For Adults and Children Who Have Nonalcoholic Fatty Liver Disease Sugar elimination can rapidly deplete the liver of its fat quite quickly. It only takes seven to eight days. That’s why Michael has a seven-day no added sugar challenge. We get so amped up on sugar, we begin to crave it more and more. Cut it out! [21:00] There Are Over 200 Names For Caloric Sugars The current count is about 260 or 270. Food companies are developing healthy-sounding names for sugar. Here are some examples: Organic brown rice syrup Evaporated cane juice Agave nectar Fruit juice concentrates Some of the fruit sugars can have even more fructose and high fructose corn syrup. Gatorade is a fake healthy drink. [23:40] What’s Happening To Children When They Consume Less Sugar Kids are more intelligent if they eat less sugar. With less sugar, kids can expect to have these benefits: Perform better on academic tests Better memory Better sleep More focus [32:15] How Much Fructose Is Really In Coca-Cola? Michael Did The Research Michael wanted to know what was in Coca-Cola. Products say high fructose corn syrup; we don't actually know what that is. The food industry says it's marginally more fructose than glucose in their products. Michael went to the supermarket and got all kinds of drinks. He sent them off to the lab. About 65% of the sugars are fructose. Michael presented this information six years ago. The food industry told him that his research was invalid. However, they couldn’t prove how. Remember, more fructose causes a lot of problems in the body. [35:25] True or False: It’s Impossible To Be Addicted To Sugar Here are some of the criteria for addiction: Withdrawal Side effects Difficult to give up If you look at the criteria for addiction, they stand true for sugar. It makes no sense that qualified professional dietitians would say that sugar is not addictive. About 40% of the Academy of Dietetics and Nutrition funding comes from big food companies. [45:15] What’s Next For Michael In 2022 Michael is wondering why certain populations have the highest rates of obesity, diabetes, and fatty liver.
