It will cause you to burn excess body fat and improve your overall health. But there’s one important caveat. A constant, unrelenting state of ketosis—in which your body is burning its own fat for fuel—is obviously unsustainable, because eventually you’d have no more fat. In this chapter, we move into the Flex pillar, which will allow you to maintain your Keto lifestyle month after month, year after year. Four Reasons Why Prolonged Ketosis Is Counterproductive My mantra is, “Lose the excess fat; treasure the necessary fat.” Ketosis is a brilliant strategy for accomplishing the first goal. As for the second goal, prolonged ketosis doesn’t have an automatic shut-off valve. If you remain in state of constant ketosis, burning your own fat, then your body will keep doing that as long as it must to stay alive. If necessary to preserve life, once your fat is gone, various bodily functions will be shut off or minimized. This process of shutting down will continue until Mr. Death knocks on your door and says, “Sorry, buddy—time’s up.” No one wants that. To ensure that you’re well informed, let’s review some of the bad things that can happen if you go too far with ketosis. Fat Burning Slows Down As levels of body fat drop, your primal brain, whom we thought we had trained to accept a high-fat diet and fasting, will be aroused and will begin to object. Why and when will this happen? Your primal brain, that eternal watchdog, will sense that your reserves of fat are becoming depleted. Remembering those long periods of deprivation thousands of years ago, your primal brain will sound the alarm: The fat supply is running low! It will issue the command that your body must preserve its precious fuel. In response, your body will actually slow down fat burning—all for the sake of survival! When you stay in ketosis too long, the body starts to rebel, and conserve its fuel. A good analogy is if you lived out in the woods and you had stored 500 logs of firewood to heat your cabin for the winter. These were your reserves, because you could still go out and chop more firewood. So if you had to dip into these reserves, you wouldn’t be worried. But now it’s early March, and snow still covers the ground. It’s been a tough winter and you haven’t harvested many trees, and you see that you’ve got only 50 logs left. That’s a very low supply! Suddenly you’re reluctant to use them because you don’t want to run out. When you had 500 logs, you didn’t care about burning them. The same thing happens with your body fat. This is why when you stay low carbohydrate for too long, your body will slow down fat metabolism. And you know what? This is a good thing. There is a big difference between losing excess fat and losing all your fat. You want to lose excess fat. But your body needs a small amount of fat, and your primal brain is smart to insist that you meet this minimal requirement. Thyroid Problems Hormone production is often a function of a “chain of command,” as orders are passed from one organ to another, until the desired outcome is achieved. The hypothalamus and the pituitary gland, which are located in the brain, help control the thyroid gland, a butterfly-shaped gland located at the base of your throat, just below the Adam’s apple. To provide their direction to the thyroid, the hypothalamus releases thyrotropin-releasing hormone (TRH), which stimulates the pituitary gland to release thyroid-stimulating hormone (TSH). When the hypothalamus and pituitary are working normally, they respond when: Thyroid hormone levels are low, so they secrete more TRH and TSH, which stimulates the thyroid to make more hormones. Thyroid hormone levels are too high, so they secrete less TRH and TSH, which reduces hormone production by the thyroid. The thyroid gland uses iodine from food to make two thyroid hormones: triiodothyronine (T3) and thyroxine (T4). The primary secretory product is inactive thyroxine (T4), which is a prohormone of triiodothyronine (T3). T4 is converted to T3 peripherally by tissues with high blood flow, including the liver and kidneys. In the brain, T4 is converted to active T3 by type 2 deiodinase produced by glial cells. The thyroid stores T3 and T4, and releases them as they are needed. These two thyroid hormones function in almost every cell in your body, and play a fundamental role in regulating your metabolism and maintaining your resting energy expenditure—that is, the energy you burn just to keep functioning while sitting on the sofa. Diet can influence thyroid hormone levels, and prolonged ketosis and severe calorie restriction can lead to reduced thyroid function, or hypothyroidism, which is a mechanism to ensure survival in response to perceived famine. The goal is to slow metabolism, preserve energy, and preserve vital organs. T4 is the inactive form of thyroid, it needs to be converted to T3 which is the active form used by the cells. Do you know which hormone helps with this conversion? Insulin. Low thyroid hormones cause a drop in levels of insulin, the hormone needed to transport sugar from the blood to different cells throughout the body. Chronic low insulin levels slow down cellular function, including muscle contractions and basic brain function. Over longer periods of time, low insulin levels compromise this T4 to T3 thyroid hormone conversion. Testosterone Deficiency Sex hormone binding globulin (SHBG) is a protein made by your liver. It binds tightly to three sex hormones found in both men and women—estrogen, dihydrotestosterone (DHT), and testosterone. SHBG carries these three hormones throughout your blood. SHBG controls the amount of testosterone that your body tissues can use. Too little testosterone in men and too much testosterone in women can cause problems. Factors such as sex, age, obesity, liver disease, and hypothyroidism can change the level of SHBG in your blood. Hypothyroidism is when your thyroid produces an insufficient amount of thyroid hormones, which can happen with prolonged ketosis. What I’ve seen reviewing lab work over the years is too much SHBG leads to lower testosterone levels, and not enough SHBG leads to an increase in estrogen; both can be problematic for men and women. Long term ketosis can lead to either of these. Leptin Reset Leptin is your “stop eating!” hormone that plays an instrumental role in your hunger and weight management. It comes from the Greek word ”leptos,” meaning “thin.” Fat cells produce leptin in proportion to levels of body fat: the more fat cells you have, the more leptin they make. When your body fat increases, your leptin levels go up. Your brain senses the increase in leptin, and thinks, “Okay, we’ve got enough fat in reserve, so we can stop eating.” Thus, you eat less and burn more. Conversely, when you don’t eat, your body fat decreases, leading your leptin levels to drop. Your brain, sensing a shortage of leptin, thinks, “Hmm, we’re low on fat reserves. Better find some food!” At that point, you eat more and burn less. This negative feedback loop is similar to the control mechanisms for many physiological functions, such as body temperature, breathing, and blood pressure. A properly working leptin system tells your brain that you have enough energy stored in your fat cells to engage in normal, everyday metabolic processes. It leads to better brain function, memory, metabolic performance, mental sharpness, and coordination, and it can even affect the regulation of mood and emotion. This is wonderful—except that many overweight people suffer from leptin resistance. They have plenty of blood leptin, but the brain doesn’t recognize it. Despite the abundance of leptin, the brain thinks there is too little leptin—and orders the body to keep eating! This is obviously a bad situation. What can cause it? Several potential mechanisms behind leptin resistance have been identified. Free fatty acids. Having elevated free fatty acids in your bloodstream may increase fat metabolites in your brain, and interfere with leptin signaling. Inflammation. Inflammatory signaling in your hypothalamus could be a cause of leptin resistance. Having consistently high leptin. It may be that your brain gets accustomed to a constant flood of leptin churned out by your fat cells, and becomes “numb” to it. So, what happens when you follow a low-carb keto diet that puts you in long-term ketosis? Perhaps paradoxically, many people do not suffer from an increase in appetite, as you might expect from falling levels of leptin. It’s complicated, but the short version of the story is that leptin levels will drop, which would normally signal your brain to start eating. But something else happens—your brain becomes more sensitive to the effects of leptin, so a smaller amount of leptin packs more punch. In addition, the brain’s response to leptin is inhibited by inflammation, resulting in increased leptin resistance. Obesity increases inflammation, which increase the brain’s “numbness” to high levels of leptin. Inflammation is dramatically reduced by sustained nutritional ketosis. The reduction in leptin resistance due to reduced inflammation adds to the brain’s heightened sensitivity to leptin. In other words, on a well-formulated ketogenic diet the brain perceives a greater satiety response to less leptin. This explains the paradox as to why we see a decrease in appetite with nutritional ketosis. In order to achieve a sustained state of ketosis, you need to occasionally boost your levels of leptin and send the signal to your brain that higher levels of leptin are normal, and that you need to keep eating. Danny found so many benefits within a week of switching that he knew this would be the new way of life. Inflammation and injuries from years of playing on turf and beating himself up with heavy lifting were gone.   Danny regained control over his hunger and found mental clarity and an increase in productivity that he had never experienced before. In August of 2017, after over a year of dipping their toes in the water, his two boys joined him. Desmond, the eldest, felt an immediate increase in energy and improvement in his allergies and asthma. Dean, the youngest, has had astounding improvements in his eczema and mood.   Danny's children are immune to cravings. It has been a fantastic thing to see. They thought their kids were "picky" just like most kids supposedly are; keto changed their taste buds, and now they crave real, nourishing food. Best of all, they do not feel restricted in any way!   In this episode, Danny breaks some common myths about the ketogenic diet – during the first three weeks of the diet, you shouldn't be lowering your protein intake. Danny also touches on the importance of salt, potassium, electrolytes, and magnesium. Then, Danny explains why he would carb load during high-performance athletic workouts. In 2013, Danny got into power lifting – making it into the top 25 in the country. When he deadlifted 700 lbs., he tore his left meniscus. In 2016, Danny’s knee was hurting like crazy, so he decided just to get healthy. He tried a typical bodybuilding diet; they did well. Eventually, he and his wife started on the ketogenic diet.   [18:25] Mistakes Made with the Ketogenic Diet There is an obsession with moderating protein. Keto is getting to a point where there is enough information out there that most people think they know what it is. People think if they overeat protein, it will turn into sugar. Danny says you can’t lower protein too much in the first three weeks. Make sure your protein is high in the first three weeks. Aim for five to eight grams of salt per day. Five grams of salt is not five grams of sodium. A teaspoon of salt will give you two grams of sodium. Get to a higher point with your metabolism so you have more to work with. Danny gets women to eat more each day because they do not eat enough. Being undernourished will impede fat loss. It’s not just cramps we should be worrying about. Potassium will help minimize overnight muscle spasms, which may contribute to you waking up in the middle of the night. Keep your electrolytes high and take a magnesium supplement.   [28:10] Should You Do a Weekly Scheduled Carb Load? Fifty grams of carbs post-workout was all Danny could give to himself. However, he knew the keto diet wasn’t taking into account high-performance athletes. A fat-adapted athlete is going to be able to eat a lot more carbs. Although, that doesn’t mean they should eat more carbs. Fat can create the sugar that we need. If you are chronically doing high-performance training, your body will be stressed without the carbs. It’s important to have carbs during days like that. Glucose intolerance can act up over time while practicing the ketogenic diet. Use carb loads when you need them; weekly might not necessarily make sense. “Everything should be done strategically.”   [34:30] Never Feed Your Kids From The Kid’s Menu This is one of Danny’s biggest pet peeves. We are doing our children such a disservice. If you are trying to conceive, take methylated b vitamins. Breastfeed your children for gut health and immune health. Nurse your babies until they start showing interest in food. Then, cut the babies food into strips. When you go to restaurants, you will see the kid’s menu with mac and cheese, chicken nuggets, and hotdogs. If both parents are together, get both parties on the same page. Talk to your partner about what is okay for your kids to eat. Get rid of all the crap in the house – have a list of non-negotiables. When you go out, have a plan. Welcome to the Keto Kamp Podcast Episode 69 with Brendan Vermiere   Today, I am blessed to have here with me Practitioner, Founder, and Owner of Metabolic Solutions LLC, Brendan Dean Vermeire. Brendan has always been an extremely motivated individual and grew up active in martial arts and wrestling among dabbling in other activities.   At a young age, he realized the importance of fitness and nutrition. Brendan felt empowered by the idea that he had control over his health and physique. Brendan enlisted in the United States Navy for the prestigious SEAL program. Unfortunately, his SEAL aspirations were cut short due to an inguinal hernia that led to a medical discharge.   Brendan transitioned into competitive bodybuilding and started personal training and nutrition coaching at the age of 19. When he started in the industry, he had the pleasure of working with experienced professionals with a vast array of backgrounds and education. Brendan was fortunate enough to gain exposure to many modalities and areas of science.   Being an extremely motivated individual, that exposure has set him on a path to learning as much as he can to optimize human health, happiness, and performance. There is nothing more rewarding to me than being able to help individuals attain a level of wellness they never thought possible.   In this episode, Brendan explains the difference between health and athletic goals. Exercise is a big piece of the puzzle; however, it isn’t the most significant piece. Then, Brendan describes how he feels to have the privilege to lecture medical doctors on functional medicine – it’s a millennial movement. Later, Brendan says that women make up a majority of his clients and reveals the mindset shift that needs to take place to lose weight and become healthy. However, there is so much in-between on becoming healthy. “You have to get healthy to get into shape.” Cater your exercise regimen to your current physiology. Fitness is essential, yet not the biggest piece of the puzzle.   [11:00] Lecturing to Medical Doctors Brendan started in fitness and nutrition. Functional medicine is an extremely popular thing right now. However, there is no such thing as a doctor of functional medicine - there is no standard of education in this field. Millennials will be leading the functional medicine movement. Enrollment into naturopathic medical school is going down. Functional medicine is a free-for-all. Brendan thinks he has a lot of valuable insight starting as a professional trainer. He sees the full spectrum and all the errors in the system.   [14:55] The Holistic Savage   Brandan finally got into the social media game. You have to put yourself out there if you want something to happen. Brandan didn’t want to be seen as a bodybuilder even though he’s done loads of bodybuilding shows. He is so much more than a bodybuilder. His goal is to elevate the vibrations of consciousness and move people towards a more healthy direction for the sake of the planet. The holistic savage has become its own brand.   [19:10] Self-Awareness about Becoming Introverted If you’re not taking care of yourself, then you can’t serve to the best of your ability. Brendan takes time to recharge. If you are in the health field, then most likely, you are a giver. You need to fill your own cup before you can fill others.   [20:50] Overcomplicating Everything Brendan is so deep into the health rabbit hole. There is a vast education gap from standard American to the complexities of holistic medicine. At the end of the day, it doesn’t need to be that complicated. We need more general applications. It wouldn’t be constructive for somebody to hyper monitor their state of ketosis. We have never had so much information readily available all the time. There is no magic formula.   [23:20] Starting with the Fundamentals FDN – functional diagnostic nutrition. Brendan doesn’t see any other schools focusing on clinical holistic health coaches. Chronic diseases are a result of our lifestyles. We need to cut down our radiation. Medication and supplements should be as needed, not mandatory.   [26:30] Women and Weight-Loss Women are more health-conscious and care more about their health. Many women suffer from hormone imbalance and weight-loss resistance. Stress and toxins drive a lot of it. Eating a standard American diet is a big issue that Brendan sees. When people think they need to get in shape – they focus on diet and exercise. Plus, they are fueling the process with self-loathing and a sense of inadequacy. Step one: fuel the health building journey out of unconditional self-love. Practice self-love more regularly. We all judge ourselves – let’s start accepting ourselves and judging less harshly. Loving ourselves enough to have the discipline to practice self-love. Create space for yourself to heal, then apply the fundamental principles. The health and fitness industry uses their marketing by asking if we are unhappy with the way we look. Sleep needs to be a priority.   [31:40] What’s Wrong with Eating Less and Moving More Brendan got so sick of his relationship with food. He went back to basics and made his life easier. He said he would eat whenever he wanted with some rules: it has to be real food. Everything comes back to the mind. We need to challenge our belief system and the story that we are telling ourselves. Start with self-awareness, self-love, and eating real food. “The mind is the most powerful thing in the body.” Counting your calories is a big distraction from the things that actually matter.   [34:50] The Relationship Between Weight-Loss Resistance and Inflammation What drives disease? Ultimately it’s inflammation and oxidative stress. They go hand-in-hand and are the destructive processes that lead to disease. Have some kind of objective assessment to determine these two variables. Most women have autoimmunity activity in their bodies. Just because you haven’t been diagnosed doesn’t mean autoimmunity isn’t occurring. Autoimmunity is a process where the immune system starts destroying our own tissues. Ultimately, it’s a gut health destruction and a trigger that’s confusing the immune system. If you add up the cases of autoimmunity – it would outrank cancer. All types of cancer are represented by “cancer.” Whereas with autoimmunity, every disease is looked at separately.   [41:35] Determining Inflammation and Oxidative Stress What’s triggering these? Environment, lifestyle, food, bugs, and toxins. “We are more microbe than human.” Brendan is working with a lab to bring on inflammation and oxidative stress test to the market. Right now, there isn’t a test to bring both of those answers at a decent price point. As we work through a health program, we should see those numbers go down. Brendan’s favorite way to test for these is on a bunch of different markers on various tests.   [45:30] Leaky Gut If you swallow dirty lake water, there are all sorts of bugs in that water. However, there should be a barrier in our gut that protects us. If that barrier is broken, it leaves us susceptible to these bugs.
