The Uberlingen Collision: Systems Failure and Human Tragedy

Investigators later determine that insufficient training and improper handling of the situation led the crew into spatial disorientation, causing a rapid and unrecoverable descent into the sea.
A separate tragedy occurs when a maintenance error causes an incorrect fuel reading to be displayed to the crew.
