Diane Pagan is a seasoned nurse practitioner and founder of Senior Moments Care, a house-call-based geriatric practice serving over 100 patients across Westchester County, NY. With more than 27 years of experience in home care and a background in complex care management, Diane combines clinical expertise with a deep commitment to culturally competent, patient-centered care. She is also a hospice-certified NP and an active mentor to other clinicians looking to break into house calls.
