#85. URGENT - Meniscal Surgery May Worsen Your Knee Pain - New Study Finds

If you've been told you need surgery for a torn meniscus, there's a brand-new, 10-year study you should see first. Dr. Clayton Dir, PT, DPT breaks down a landmark sham-surgery-controlled trial that followed adults with degenerative meniscal tears for a full decade — and the results challenge what a lot of people are told about needing surgery.


The findings: no meaningful benefit from surgery over leaving the meniscus alone, and the surgery group actually had worse long-term outcomes, including a higher rate of new or worsening knee osteoarthritis. But this episode isn't "surgery is always bad" — it covers exactly where surgery still matters, and where it doesn't.

Is this true for all meniscus tears?
No. This applies specifically to degenerative meniscal tears in adults. It doesn't apply to cases where a torn piece is mechanically locking the knee, where surgery can still restore motion.

What should I do instead of surgery for a meniscus tear?
Nonsurgical approaches like physical therapy, targeted strengthening, and addressing mobility can be a legitimate first step — this isn't medical advice for your specific case, but a reason to ask more questions before agreeing to surgery.
