Today we welcome a specialist in Weight Management and Bariatric Surgery for adults and adolescents, Dr. Robert Cywes. He has been doing bariatric surgery for 18 years, performing over 8,000 surgeries. Dr. Cywes’ medical training began in Cape Town, South Africa, where he received his medical degree from The University of Cape Town. In 1989, Dr. Cywes moved to North America and completed a year-long residency in Pediatric Surgery at Ohio State University’s Columbus Children’s Hospital. After completing his pediatric surgery fellowship at the University of Michigan’s C.S. Mott Children’s Hospital, Dr. Cywes was appointed as an Assistant Professor of Pediatric and Fetal Surgery at Vanderbilt University in Nashville, Tennessee where he did hepatic stem cell research. During this time, Dr. Cywes became increasingly interested in adolescent obesity and the impact of the liver and metabolic syndrome on young patients. Dr. Cywes’ research led to a comprehensive understanding of the toxicity of chronic excessive carbohydrate consumption as the primary cause of obesity and so-called obesity-related co-morbidities, and he became interested in developing a clinical program to treat obese patients using this knowledge. Dr. Cywes relocated to Jacksonville, Florida where he joined the Department of Pediatric Surgery at the Nemours Children’s Clinic and Wolfson Children’s Hospital. This led to a national meeting in Jacksonville where guidelines for adolescent obesity surgery were established. Dr. Cywes established JSAPA to continue his work in both adolescent and adult obesity treatment and surgery, and in 2013 opened a practice in Palm Beach County, Florida. He now works with a highly experienced team of professionals from a variety of medical sub-specialties to better care for obese patients. He has developed the practice into an internationally recognized Center of Excellence for obesity surgery. The practice uses a cognitive behavioral therapy approach that addresses carbohydrate addiction, along with bariatric surgery, to help patients manage their obesity long term. Addicts are usually not able to see how bad it is within themselves. He moved to Canada, where he completed his general surgery residency and specialized in Minimally Invasive Surgery at the University of Toronto. Also, Dr. Cywes earned a Ph.D. in Liver Carbohydrate Metabolism and the effect of glucose metabolism on liver transplant immunology. Cywes learned that the body doesn’t need to run on sugar. In fact, it prefers a keto method of running. He also learned that fatty liver doesn’t come from the consumption of fat. It happens from infusing insulin and sugar. [10:00] About LDL LDL is a complex molecule. It’s a truck on the road transporting fat on your body. LDL will lay down fat over an injury. HDL will come along and remove the fat. LDL is not causing the injury; it’s desperately trying to heal the injury. Think of the liver like a subdivision. You have all the houses which are the different cells. Tiny roads that go to each house. Think about a bunch of garbage that blocks the road. You can’t get food and oxygen downstream to those cells. Eventually, those cells will die. As you infuse sugar into the liver, you will see that swelling. [14:15] The Difference Between Ketosis and Diabetic Ketoacidosis The difference between ketosis and diabetic ketoacidosis is 5.5 – 6 grams of sugar. It’s half to one gram difference. It’s an incredibly small difference. An apple has 25 grams of sugar in it. That’s a five-fold increase in blood sugar. Think about how resilient the body is to take sugar out of the bloodstream. [16:15] Flaws With Conventional Approaches to Diabetes Nothing fixes diabetes. Drugs just slow down the process. Once someone goes on the drugs, they are told to eat sugar. There is a misconception that the body has to have sugar. If you don’t have carbs going in, then you are not going to have the type 2 effect. If you manage your blood sugar tightly, you don’t have insulin resistance. Chronic carbohydrate consumption will cause resistance. If the body protects itself by resistance, then it doesn’t matter what the source of the insulin is. A1C is an injury to blood cells because they are damaged. [20:35] Effectively Treating Diabetes If we are going to treat diabetes effectively, we have to understand how our body protects itself. We can treat diabetes without sugar. We are relying on people who are dependent on the diabetic manufacturing industry to tell us not to eat sugar. It’s counter-productive. How do you tell people that statins are unnecessary? No one can get their hands on an inexpensive CGM. You cannot manage a type 2 diabetic without a CGM. Yet, because of the FDA, these things are so expensive that people cannot afford them. CGMs safe lives. [23:40] The Best Feedback Instrument to Lose Weight The best instrument to lose weight is a CGM. The worst device to lose weight is a scale. You can eat a tub of ice cream, and the scale will go down. You can do a 24 hour fast, and the scale will go up. As soon as you put the first spoon of ice cream in your mouth, your CGM will be alarming. [26:30] Our Bodies Have Feedback Controls for Survival For every action, there is an equal and opposite reaction. If something is not necessary for human survival, then it doesn’t have a feedback system. The systems are very complex at the interface level. The human body will use one molecule to create a whole variety of downstream effects. It is ridiculous to tell people to drink water. Your body knows when it needs water - it will rarely allow yourself to get thirsty. There is no need for a mathematical formula to tell us when to drink. Alcohol is not necessary for survival. There is no stopping point for our bodies. That is why we have a mathematical formula for when to stop drinking. The human body has no need to know when you should stop eating. There is no feedback control for the consumption of carbohydrates. People are not fat loading their food, they are fat overloading. It’s not okay to eat a lump of butter with steak attached. Overeating fat will make you feel like crap afterward. Part of keto is allowing your body to tell you when it has had enough. [40:30] Keto is a Metabolic State There is so much bullshit in nutrition science. The best place to go to is understanding the complexity of human biology. Human beings changed to become better with our brains. To develop brain function, we needed a more significant source of animal fat. We hunted and primarily lived on coastlines with easy access to water and seafood. Our brains need animal fat. Human beings evolved to become omnivores. Our primary focus is on the carnivore diet when we don’t have access to animal fat. If you have to supplement your food with pills, then you are not on a complete diet. [48:30] Dopamine Fasting and Serotonin Loading Carbohydrates are a drug and not food. When you get rid of carbohydrates, they will leave deficits. People don’t understand that the main reason they overuse carbohydrates is because they have a dysfunctional relationship with a substance that gives them instant gratification. If you have a rough day and eat a tub of ice cream; then you’ve had instant gratification. Going on a walk requires effort. However, it allows you to process tough issues that drive emotions. A healthy person can focus for 20 minutes, then they give themselves a serotonin break like a sip of coffee or a deep breath. We have lost track of effective serotonin relief. Today, I am lucky to have here with me, the CEO and co-founder of the Fasting Method Program, clinical educator, and researcher, Megan Ramos. Megan’s major role in the The Fasting Method is to facilitate the patient’s education on the Fasting Method dietary principles and counsel them on their fasting regimens. Megan, along with her colleague Dr. Jason Fung, believe that both what you eat and when you eat matter. The Fasting Method utilizes therapeutic fasting and time restricted eating protocols with their patients to help them reverse their type 2 diabetes and achieve sustainable weight-loss. They encourage their patients to eat real foods and adopt a low-carb lifestyle in addition to the fasting. Megan was diagnosed with non-alcoholic fatty liver disease (NALFD) at age 12, polycystic ovarian syndrome (PCOS) at age 14, and type 2 diabetes (DMII) at age 27. She reversed these metabolic conditions and achieve an 86-pound weight loss by incorporating intermittent fasting with a low carb diet, and is now passionate about helping others do the same. Megan lives in Toronto, Canada. She is a director of the nonprofit organization Public Health Collaboration Canada (PHC Canada), works on the editorial board of the Journal of Insulin Resistance and Team Diet Doctor. She was diagnosed with fatty liver disease, polycystic ovarian syndrome, and type 2 diabetes. She was skinny, but all of her weight was fat. Her doctor couldn’t tell why she had diseases of obesity. Megan didn’t stress about it. “Your genes aren’t your destiny.” –Megan Ramos Megan started working with Dr. Jason Fung at age 15. Her research patients were trying to live life right; however, they were just gaining more weight. Megan also was gaining weight despite hiring a nutritionist and personal trainer. When Dr. Fung would introduce fasting to his patients, they would feel better. It was the missing piece of the puzzle. Megan started fasting, and it finally all made sense.   [13:10] Symptoms for Kidney Dysfunction Patients who are leaking a lot of protein in their urine, yet they don’t have high blood pressure are most likely suffering from kidney dysfunction. Protein in the urine usually means something diabetic is going on. “In North America, you have insulin resistance until proven otherwise.” –Megan Ramos Even though your labs say normal, a conventional doctor will not be able to read the labs as an expert would.   [17:30] Being Proactive Rather Than Reactive Megan wants to help patients be proactive. The healthcare system is broken beyond repair. When Megan and Jason first started talking about fasting, people thought they were lunatics. However, the benefits of fasting have been well known for hundreds of years. Fasting makes a significant change in the lives of their patients.   [23:40] Fasting the Wrong Way Fasting the wrong way will result in electrolyte imbalance. Most people are petrified of salt. However, salt is critical to our survival and wellbeing. Insulin levels will be high when you first start fasting. Eventually, the insulin levels will tank. It could cause diarrhea if not rehydrating correctly. Megan does consultations online. Most of the time, the person’s issues are resolved by salt or magnesium. Pickle juice and bone broth will also assist with hydration. Sometimes people carb up before a fast. It will wreak havoc on your body and mind. Plus, you won’t get autophagy as efficiently. Do a fast through your ketogenic diet. Fasting shouldn’t make you feel bad; it should make you feel more energetic.   [31:40] The Most Surprising Benefit of Fasting Megan loves when older women get their libido back. The most surprising thing is anti-aging in women. Women who have gone grey are now getting their hair color again. A 76-year-old woman got her regular menstrual period back for two years.   [33:50] Should Women Practice Fasting Differently Than Men? Women need to be more diligent with sodium. Salt is critical in middle-aged women. Not necessarily for feeling good, but for making sure that weight loss is happening at an efficient rate. Women need to fast more than men to get the same results. A 24 hour fast will give women tremendous results. They will lose water weight and body fat. Women are more compliant doing a three day fast. Once they wake up on the second day, there should be no hunger. Staying in a fast can be more manageable for women.   [37:55] Advice for Someone Who is Getting Stuck at a Certain Weight Make sure there are no problem foods or snacking in the diet. People on keto will eat a lot of nuts and cheese outside of eating windows. What do your eating days look like? Add fat and protein to your meals to make them more satiating. That alone can break a person through a plateau.   [42:55] Megan’s Favorite Benefit of Fasting Megan gets an insane amount of energy while fasting. She gets to train doctors and speak at medical conferences. After a trip, Megan does a fat fast. Mercury is a toxin that causes all sorts of problems in our bodies. She started having health problems nine months after getting breast implants. Her mercury bucket was full. Sometimes when you are young, you can have mercury symptoms that won’t really affect you. Mothers can pass mercury to the fetus as a defense mechanism from their bodies. When we ingest mercury from our diet, it can end up in muscle tissue.   [06:30] Symptoms after Breast Implants   After getting breast implants, Dr. Kayte would become extremely sick after drinking alcohol. Plus, her brain wasn’t working as it should – she had brain fog and tons of post-workout soreness. She was sent on a whole health journey which led her to become a chiropractor. She knew something was wrong, and she was in pain. During the first term of chiropractor school, she realized it wasn’t helping her pain. So, she started learning about natural health and functional medicine. Autoimmunity has a genetic component – they often call it a lifestyle disease because there are unknown reasons why it can turn on. Dr. Kayte dismissed this as a cause. After doing more research, Dr. Kayte found out all the ingredients in breast implants, including mercury. After learning this, Dr. Kayte didn’t want her breast implants.   [21:00] What to do Before Getting an Explant   You don’t need to start detoxing if you still have implants. Support the body and the healing process before an explant. An explant is a massive trauma. Get a lot of vitamin C and zinc. Work with your doctor and tell them that you are focusing on healing. Women do not have a lot of information about this in the mainstream media. These types of conversations are not happening enough.   [23:30] If Someone Feels Fine With Their Breast Implants   Kayte says it’s good that you feel fine. She would want them to be educated about what is actually in the implants. Think about what’s going to happen when it’s time to replace your implants. The plastic surgeon is going to try and replace them with the most cohesive implant. The most cohesive implants have the most toxins and chemicals in them.   [25:40] What to do After Getting an Explant   If you’re feeling terrible, there’s a chance the chemicals have affected your brain. Measure your mineral status and interpret it with your doctor. You can improve estrogen dominance and balance your hormones. Supplement things to make serotonin. Start thinking about chelation.   [28:40] Testing for Mercury There are many different ways to test for mercury. Mercury has the ability to block itself into your body. Not everyone stores mercury in the same way. Sometimes, you can have an exposure, and you can clear it. If you are storing mercury, you will probably not have high levels in your blood test. The hair test will not test for heavy metals; it will test for mineral derangement. You can test how the hair in your body is utilizing minerals.   [36:45] Dr. Kayte’s Favorite Health Tool Fasting is an anti-aging tool, and it resets everything in our bodies.
