Today we welcome a specialist in Weight Management and Bariatric Surgery for adults and adolescents, Dr. Robert Cywes. He has been doing bariatric surgery for 18 years, performing over 8,000 surgeries. Dr. Cywes’ medical training began in Cape Town, South Africa, where he received his medical degree from The University of Cape Town. In 1989, Dr. Cywes moved to North America and completed a year-long residency in Pediatric Surgery at Ohio State University’s Columbus Children’s Hospital. After completing his pediatric surgery fellowship at the University of Michigan’s C.S. Mott Children’s Hospital, Dr. Cywes was appointed as an Assistant Professor of Pediatric and Fetal Surgery at Vanderbilt University in Nashville, Tennessee where he did hepatic stem cell research. During this time, Dr. Cywes became increasingly interested in adolescent obesity and the impact of the liver and metabolic syndrome on young patients. Dr. Cywes’ research led to a comprehensive understanding of the toxicity of chronic excessive carbohydrate consumption as the primary cause of obesity and so-called obesity-related co-morbidities, and he became interested in developing a clinical program to treat obese patients using this knowledge. Dr. Cywes relocated to Jacksonville, Florida where he joined the Department of Pediatric Surgery at the Nemours Children’s Clinic and Wolfson Children’s Hospital. This led to a national meeting in Jacksonville where guidelines for adolescent obesity surgery were established. Dr. Cywes established JSAPA to continue his work in both adolescent and adult obesity treatment and surgery, and in 2013 opened a practice in Palm Beach County, Florida. He now works with a highly experienced team of professionals from a variety of medical sub-specialties to better care for obese patients. He has developed the practice into an internationally recognized Center of Excellence for obesity surgery. The practice uses a cognitive behavioral therapy approach that addresses carbohydrate addiction, along with bariatric surgery, to help patients manage their obesity long term. Addicts are usually not able to see how bad it is within themselves. He moved to Canada, where he completed his general surgery residency and specialized in Minimally Invasive Surgery at the University of Toronto. Also, Dr. Cywes earned a Ph.D. in Liver Carbohydrate Metabolism and the effect of glucose metabolism on liver transplant immunology. Cywes learned that the body doesn’t need to run on sugar. In fact, it prefers a keto method of running. He also learned that fatty liver doesn’t come from the consumption of fat. It happens from infusing insulin and sugar. [10:00] About LDL LDL is a complex molecule. It’s a truck on the road transporting fat on your body. LDL will lay down fat over an injury. HDL will come along and remove the fat. LDL is not causing the injury; it’s desperately trying to heal the injury. Think of the liver like a subdivision. You have all the houses which are the different cells. Tiny roads that go to each house. Think about a bunch of garbage that blocks the road. You can’t get food and oxygen downstream to those cells. Eventually, those cells will die. As you infuse sugar into the liver, you will see that swelling. [14:15] The Difference Between Ketosis and Diabetic Ketoacidosis The difference between ketosis and diabetic ketoacidosis is 5.5 – 6 grams of sugar. It’s half to one gram difference. It’s an incredibly small difference. An apple has 25 grams of sugar in it. That’s a five-fold increase in blood sugar. Think about how resilient the body is to take sugar out of the bloodstream. [16:15] Flaws With Conventional Approaches to Diabetes Nothing fixes diabetes. Drugs just slow down the process. Once someone goes on the drugs, they are told to eat sugar. There is a misconception that the body has to have sugar. If you don’t have carbs going in, then you are not going to have the type 2 effect. If you manage your blood sugar tightly, you don’t have insulin resistance. Chronic carbohydrate consumption will cause resistance. If the body protects itself by resistance, then it doesn’t matter what the source of the insulin is. A1C is an injury to blood cells because they are damaged. [20:35] Effectively Treating Diabetes If we are going to treat diabetes effectively, we have to understand how our body protects itself. We can treat diabetes without sugar. We are relying on people who are dependent on the diabetic manufacturing industry to tell us not to eat sugar. It’s counter-productive. How do you tell people that statins are unnecessary? No one can get their hands on an inexpensive CGM. You cannot manage a type 2 diabetic without a CGM. Yet, because of the FDA, these things are so expensive that people cannot afford them. CGMs safe lives. [23:40] The Best Feedback Instrument to Lose Weight The best instrument to lose weight is a CGM. The worst device to lose weight is a scale. You can eat a tub of ice cream, and the scale will go down. You can do a 24 hour fast, and the scale will go up. As soon as you put the first spoon of ice cream in your mouth, your CGM will be alarming. [26:30] Our Bodies Have Feedback Controls for Survival For every action, there is an equal and opposite reaction. If something is not necessary for human survival, then it doesn’t have a feedback system. The systems are very complex at the interface level. The human body will use one molecule to create a whole variety of downstream effects. It is ridiculous to tell people to drink water. Your body knows when it needs water - it will rarely allow yourself to get thirsty. There is no need for a mathematical formula to tell us when to drink. Alcohol is not necessary for survival. There is no stopping point for our bodies. That is why we have a mathematical formula for when to stop drinking. The human body has no need to know when you should stop eating. There is no feedback control for the consumption of carbohydrates. People are not fat loading their food, they are fat overloading. It’s not okay to eat a lump of butter with steak attached. Overeating fat will make you feel like crap afterward. Part of keto is allowing your body to tell you when it has had enough. [40:30] Keto is a Metabolic State There is so much bullshit in nutrition science. The best place to go to is understanding the complexity of human biology. Human beings changed to become better with our brains. To develop brain function, we needed a more significant source of animal fat. We hunted and primarily lived on coastlines with easy access to water and seafood. Our brains need animal fat. Human beings evolved to become omnivores. Our primary focus is on the carnivore diet when we don’t have access to animal fat. If you have to supplement your food with pills, then you are not on a complete diet. [48:30] Dopamine Fasting and Serotonin Loading Carbohydrates are a drug and not food. When you get rid of carbohydrates, they will leave deficits. People don’t understand that the main reason they overuse carbohydrates is because they have a dysfunctional relationship with a substance that gives them instant gratification. If you have a rough day and eat a tub of ice cream; then you’ve had instant gratification. Going on a walk requires effort. However, it allows you to process tough issues that drive emotions. A healthy person can focus for 20 minutes, then they give themselves a serotonin break like a sip of coffee or a deep breath. We have lost track of effective serotonin relief. Ashleigh’s metabolism was so screwed up. Her body did not know what to do with food; she ended up gaining sixty pounds while eating nutritiously. Ashleigh hated her body and felt like she needed a new one. However, she realized she wouldn’t be able to heal if she hated her body. Her body is the only thing that is keeping her alive. Recognize the incredible gift of life that your body gives you. “It came down to reverence for life first.” -Ashleigh Di Lello   [18:50] Practicing Love and Gratitude Our subconscious does a majority of our thinking. The older we are, the more it can take over. Say affirmations like “I love my body.” These mantras will help fight the subconscious thinking. Another thing you will need to do is be more aware of those subconscious thoughts. When is the first time you decided that you hate your body? Realize that you can change those thoughts in your head.   [26:45] Stop Counting Calories   Counting calories is outdated information. It came out before we even understood our bodies and food. If you eat excess calories, the body will gain weight. However, it’s outdated. What we eat matters so much more. It will affect how we gain fat and muscle. In the dancing world, many people have eating disorders. Dancers will starve their bodies and also put poison in it. The body is designed to run with the right fuel. Calories keep people from understanding about serving our bodies with the right type of fuels. People eat fake food for the sake of their calorie allotment. If your food doesn’t have calories, then it’s not food.   [33:30] About Ashleigh’s Programs Ashleigh has been studying the brain for the past three years. She has two different coaching programs. He went from 170 lbs to 130 lbs and had a chronic gut infection. Dr. Jockers started to listen to his body and only ate when he was hungry. At this point, he started playing with intermittent fasting. Jockers developed skin cancer. It was a wake-up call to create balance in his life. After doing some research, Dr. Jockers found the ketogenic diet. Eventually, his skin cancer went away after implementing the ketogenic diet and intermittent fasting.   [07:00] The Benefits of Fasting Jockers is trying to maintain his weight and muscle mass. It is easier for him to maintain his muscle mass with fasting. Dr. Jockers likes to fast for brain function – it allows him to think sharply and quickly. Fasting will also help you feel stronger when lifting. Plus, there are mental and emotional benefits of fasting. Jockers doesn’t eat while traveling – there is no need for it. It’s hard for Dr. Jockers to fast through lunch. He fasts through dinner quite often. Having a break during the day is a ritual for most of us. We are addicted to our rituals, and our rituals revolve around food. “The quality of our lives is all about building resiliency.” -Dr. Jockers   [10:50] Fasting Fights Cancer If we are continually stimulating insulin in our body, then we are unable to heal. All of our cells need repair; they are damaged by stress, so we need a regular repair process. We need to take our damaged cells and repair them. It is more energy efficient to repair cells, then replace cells. Cancer is driven by the constant signal to grow. When we remove the signal to grow, then the body can repair. People are worried that fasting will remove muscle. However, the body will get the signal to stay strong and fast. Fasting will result in an increase in strength. Once you get going to the gym, you will feel just as strong if not stronger. Training is the number one stimulus for muscle growth.   [15:50] Does Glucose or Fat Produce More Energy? Glucose is the fastest process. Burning fat is going to take a lot longer. Burning ketones and fatty acids will produce more energy with fewer waste. If we have carbs, we will want to burn those fast. Being metabolically flexible is the ability to switch back and forth. Without food, our body can burn fat and ketones for fuel.   [19:15] The Best Foods to Eat on a Keto Diet Diets need to be customized. People do the best with grass-fed meats, organic eggs, avocados, olives, olive oils, green vegetables, dark chocolate, nuts, seeds, lemons, and limes. You can get most of the benefits of fruits from lemons and limes. Plus, the lemons and limes do not have sugar. Sour foods are good for stimulating bile and stomach acid. Most people have no idea how to stimulate these things. If you are metabolically flexible, then berries are a great source. Consuming too many berries will not allow your body to go into ketosis. We are not getting enough fat-soluble vitamin A. You can find in egg yolk, grass-fed butter, and organ meat. We should do things that stimulate our stomach acid and bile. Bile will sterilize the small intestine.   [26:50] The Biggest Mistake on the Ketogenic Diet Not including enough herbs is the biggest mistake on the ketogenic diet. Often, people are eating too many dairy products and not enough herbs. Some people feel better with less vegetables. However, they will still do well with ginger tea and herbs with their meats.   [28:00] What to do When You Are Plateauing Find out why you are plateauing. Are you actually getting into ketosis? When you are in ketosis, you should not have cravings. If you are not feeling energetic in ketosis, you may have a thyroid issue. Are the outer-third of your eyebrows thinning? This is a common sign of thyroid problems. Test your fasting insulin. You want to see it between two and five. Check for food sensitivity – you may have a sensitivity to coffee, eggs, or dairy. You can also do your own testing by putting something on your tongue and measuring your pulse. Some people need more stress on their system. Intermittent fasting will ramp up fat burning. Exercise will also help. Today I have here with me, T.J. Robinson, aka "The Olive Oil Hunter ®," is one of the world's most respected authorities on all matters olive oil. Known for his “platinum palate,” he is one of the few Americans invited to serve as a judge in prestigious Italian olive oil tasting competitions.   These days he is dedicated to importing rare fresh-pressed olive oil, the most flavorful and healthiest extra virgin olive oil on the planet, until now virtually impossible to obtain year-round in the U.S. All his oils are independently lab tested and certified for 100% purity.   T.J. serves as a personal “olive oil sommelier,” traveling the world, following the sun, seeking out only the best-tasting oils and acquiring them for his Club members right during the olive harvest, as the olive presses are running, when the oils are miraculously flavorful —like no other olive oil you can find in the U.S.   In this episode, T.J. opens the show describing the difference between his olive oil and mass-produced oils. The highest quality olives should still be green, whereas bulk producers will use overripe olives because of the high oil content. Then, T.J. explains the process he goes through to harvest olives and how much work goes behind creating his blends. Robinson J. joined a harvest party with a Sicilian family in Italy. The moment T.J. tasted fresh olive oil, it changed his whole life. Fresh olive oil could change the world, people’s diet, and their health.   [04:30] The Difference Between T.J.’s Olive Oil and Mass-Produced Olive Oil Bulk oil is made up of oils from different countries. When you look at the label, it may have five or six different countries on it. There is very little care with these oils. Some bulk oils can sit in vats for years and years before ever making it to your pantry. J. works with small family farmers who do not have access to the United States market. It is a highly curated collection, following a seasonal harvest. After trying hundreds of oils, T.J. makes test batches and eventually sends the oils out to his club members. The health qualities cannot be compared between T.J.’s olive oil and bulk olive oils.   [08:20] About the Olive Harvest There are over five hundred different olive varieties in Italy. After playing with the varieties, T.J. creates blends. The ripeness of the olives will alter the flavor profile. As the fruit goes from green to black, more oil is created inside the fruit. If the farmer is producing oil for bulk oil, they will leave it on the tree for an extra few months. During this time, it goes from 10% oil to 30% oil. The health-promoting qualities of olive oil are the greatest when it’s super green and not as ripe. Harvesting the fruit when it’s green is not as cost-effective as harvesting it when it’s overripe. Bulk producers harvest when it’s overripe because it produces more oil.   [17:30] Staying Satiated   Olive oil is one of the best fats for staying satiated. Ben will take a shot of olive oil as a crutch when fasting. J.’s olive tastes so good that you can drink it out of a shot glass. The media is finally picking up on the good fats in olive oil. They realize the younger olive oils are better.   [21:00] The Myth About Cooking With Olive Oil Myth: you shouldn’t cook with olive oil. The antioxidants in fresh oil protect it from oxidizing, whether it’s in the body or the frying pan. J. heats his pan first, at the very last minute, T.J. will add his oil. Do not let the oil sit and burn.   [26:40] Taste-Testing Olive Oils Professional taste-testers drink olive oil out of white cups. The color is an indicator of quality. Quality olive oils will have a green tint. Then, swirl the oil and put the cup in the palm of your hand to warm the oil. The first evaluation of the oil will be on the nose. When it is extra-virgin, the oil goes through a professional tasting panel and a chemistry panel. Take a whiff of the oil. You should notice that it smells like being in a vegetable garden. After you have smelled the oil, it is time to place a teaspoon of oil on your palette and chew the oil. T.J. tries to draw a little air in when he tastes. During this, you are looking for bitterness and spiciness. You should feel a tingle in your throat.   [35:20] Uses for Olive Oil Ben takes a shot of olive oil to stay satisfied. He also uses it for eggs, salad dressings, and sautéing vegetables J. likes to use it as a finishing oil. It would be best if you used it as a sauce and can use it to roast vegetables. Especially during first 28 days of keto  (hormones that satiate you) CCK Cholecystokinin, Glucagon-Like Peptide-1 (GLP-1) Glucagon-like peptide-1 (GLP-1) is a hormone produced in your gut when nutrients enter the intestines.
